Mantis kept her secret hidden for years to protect Quill from the painful truth about his father Ego. Since Mantis is Ego's daughter, revealing their sibling relationship would have forced Quill to confront the reality that Ego manipulated and used both of them. Mantis wanted to spare him from the shadow of Ego's misdeeds.

Peter Quill is still grieving the loss of Gamora, who died in Avengers: Infinity War. Although an alternate timeline version of Gamora exists, she did not share the same relationship with Quill, leaving him emotionally devastated. This depression is why Mantis and Drax wanted to find him a special Christmas present.
