The movie takes place in Hyderabad's Old City, a dense and labyrinthine area with crowded lanes and markets surrounding the iconic Charminar. Izzat refers to the code of honor that drives decisions and loyalties in Hyderabad's Old City. Even a small slight can trigger a dangerous chain of confrontations, and characters constantly weigh risk against pride as alliances shift and reputations hang in the balance. The social code functions as both a weapon and a shield.

Ismail Bhai (played by Dheer Charan Srivastav) is a powerful local gang leader in Hyderabad's Old City who commands a loyal but wary following. His world runs on honor, fear, and a calculated balance of generosity and threat. Any insult to izzat is treated as a direct challenge to his control.

The film explores three main themes: Honor (izzat), which drives decisions and loyalties in the old city; Power, which is negotiated through gossip, threats, and strategic moves rather than formal authority; and Identity, which examines the tension between diaspora aspirations of NRIs from New Jersey and the traditional loyalties and codes of Hyderabadi street politics.

The NRIs (Pranai and Rochak) from New Jersey inadvertently cross paths with local leader Ismail Bhai upon returning to Hyderabad. Ismail enlists Salim to resume an old vendetta against them, triggering a renewed pursuit and escalating conflict. The NRIs must confront the consequences of their past and fight to protect themselves from the looming danger.

Pranai (Kuntaa Nikkil) and Rochak are NRIs from New Jersey who travel to Hyderabad to join a friend's company. Pranai carries hopeful, modern ambitions and must navigate the old city's dangerous chessboard of izzat and power through quick thinking and strategic risks. Rochak provides street-smart savvy and acts as a foil to the Hyderabadi codes.

Ramesh (Raghu Karumanchi) is Pranai's cousin and loyal butler who secretly allies with Mama, a veteran Hyderabadi underworld figure. He orchestrates a kidnapping plot, revealing how even the closest circle can harbor treachery and complicate loyalties in the old city's web of power and honor.

Salim 'Pheku' (Mast Ali) is a key member of Ismail Bhai's crew, known for confidence and bravado. He embodies the street code, offering blunt loyalty and readiness for confrontation. His presence underscores the faction's readiness to defend its reputation at all costs.
