Lee Geum-hwa and her twin sister are born under tragic circumstances. The sister's fetus feeds on Geum-hwa's leg during pregnancy, leading to Geum-hwa being born with a deformity.

Shortly after their birth, their mother tragically passes away. Devastated by this loss, their father commits suicide, leaving Geum-hwa orphaned.

Geum-hwa is taken in by her grandparents who have recently relocated to a remote village. There, they lead a quiet life raising dogs, while Geum-hwa must grapple with her isolation and physical challenges.

The twin sister, referred to as 'the thing,' is kept hidden away in a shed at the family's home. Her existence is concealed from the community, and she is not even registered as a birth.

Local villagers, concerned about strange happenings on the farm, enlist the help of a shaman to investigate. Their curiosity leads them to the shed, where they encounter terrifying snakes that react aggressively.

Pastor Park is hired to delve into the mysterious cult known as Dongbanggyo or 'Deer Mountain.' His investigation coincides with the police's detailed examination of a girl's body found buried in concrete.

Gwangmok visits truck driver Kim Cheol-Jin, who confesses to being tormented by the spirits of dead children. This chilling encounter sets off a chain of events leading back to the investigative efforts surrounding the cult.

As Pastor Park investigates the cult further, he uncovers a connection to Kim Pungsa, who wrote the scriptures for Dongbanggyo. Notably, he learns that Kim Je-seok, the cult's founder, has a disturbing history with juvenile delinquents.

Gwangmok, now obsessed, stakes out Geum-hwa as he believes she is linked to the cult's sinister activities. His nightmares of dead girls grow more vivid, suggesting a deeper connection to Geum-hwa's past.

In a moment of fate, Gwangmok encounters Geum-hwa but is startled when the sister pulls his leg from under the door, causing him to flee. This unexpected event foreshadows the sister's emerging influence.

Gwangmok is led by 'Father' to a shocking discovery regarding the nature of the supposed 'snake.' Mysteries unravel as he learns that Geum-hwa herself has a role in the ongoing terror surrounding the cult.

Gwangmok kidnaps Geum-hwa, claiming that she is inherently evil and her death is predestined. In a desperate plea, Geum-hwa reveals her sister's existence in hopes of achieving redemption.

Confrontation leads Gwangmok to the sister, who has transformed and now appears as a serene figure. In a surreal moment, she prophesies to him about a six-fingered mark on 'Father,' linking him to the cult.

As Gwangmok learns the truth about his Father figure being a mere disciple, he faces betrayal from the Henchman and ultimately meets his demise. In the climax, Gwangmok ignites flames of revenge and justice.

In the aftermath, Geum-hwa clings to her sister as she fades away, marking a poignant moment of loss. Their tragic bond resonates deeply, symbolizing the cycle of life and death intertwined with their fates.
