Jens, a 16-year-old boy, starts a camping trip with friends to a cabin. They arrive and settle in, setting the scene for the events to follow. During their stay, Jens's feelings for Lisa are evident as she was from a previous summer, and she is now back due to her father's job in Oslo.

During a game of Truth or Dare, Jens is dared to kiss Lisa, which leads to a romantic moment. The two then have sex for the first time, marking a significant development in Jens's relationship with Lisa. This event also introduces the theme of intimacy in the story.

Inside Jens' testicles, a society of sperm cells is depicted, with Simen Sprut, a young and ostracized sperm, feeling disconnected due to his disinterest in the race to fertilize an egg. His friends, including Cumilla, try to motivate him to participate.

Jizzmo, an entrepreneurial sperm cell, develops the Ejaculator 9000, a powerful armored suit that enables him to fight system obstacles and other sperm. He demonstrates his strength by brutally killing a worker sperm, emphasizing his aggressive nature.

During Jens' first ejaculation, Jizzmo opens the condom, allowing himself and fellow sperm to enter Lisa’s vagina. Simen and others are also introduced into her reproductive system at this point, beginning their journey towards the egg.

Jens and Lisa have sex again, but Jens is tired and injured, leading to penis penetration through Lisa’s anus. Sperm and other characters navigate through Lisa’s urethra, bladder, and rectum, facing various challenges and dangers along the way.

Lisa applies spermicide, killing many sperm including Professor Saltsmak. Meanwhile, a gang of gangster sperms led by Egon Olsæd tries to get past the spermicide to reach the egg, leading to violent confrontations and Jizzmo’s appearance.

Jizzmo fights to kill the remaining sperm but is stopped by Egon, who causes his suit to self-destruct, sacrificing himself and allowing the other sperm, including Simen and Cumilla, to reach the egg. The sperm successfully approach fertilization.

Jens and Lisa walk together, and Lisa expresses her frustration about not achieving orgasm. They have sex against a tree, leading to their ejaculation, which causes a beehive to fall on Jens’ pelvis, ensuing chaos as they fall down a hill.

The turbulence from the fall causes Lisa’s egg to bounce and kill most sperm, leaving only Simen and Cumilla. Jizzmo attacks, but is ultimately crushed by the egg after biting off Cumilla’s tail, ensuring only the strongest sperm enter the egg.

Simen and Cumilla enter the egg together, marking successful fertilization. This pivotal moment signifies the culmination of the sperm’s journey and the beginning of pregnancy.

Jens and Lisa visit a clinic to get an abortion pill. The gynecologist explains the process through a song, advising them to consider their future planning before proceeding. The scene highlights the decision-making process regarding pregnancy.

The film concludes with Jens suggesting a movie marathon with Lisa, as the abortion pill will take two days to work. It leaves their future open, emphasizing their relationship and upcoming choices amidst the sexual and reproductive themes.
