Al Simmons, a skilled military soldier and assassin, is betrayed by Jason Wynn during a dangerous operation in North Korea. Wynn conspires with Jessica Priest to orchestrate a deadly gas fire, leading to Simmons' fatal end.

After his death, Al Simmons arrives in Hell, where he encounters Malebolgia, the Devil. Malebolgia offers Simmons a tempting deal: in exchange for serving as the leader of his army during Armageddon, he can return to Earth to see his beloved fiancée, Wanda.

Resurrected by Malebolgia's pact, Simmons returns to Earth only to discover that five years have passed. During his absence, Wanda has moved on, marrying his best friend, Terry, and starting a family he never knew he desired.

In his new reality, Al Simmons meets The Violator, a bizarre clown-like demon. The Violator begins to lead Simmons down a dark path as he grapples with his new existence and powers.

Jason Wynn has reinvented himself as a prominent arms dealer while developing a catastrophic biological weapon named 'Heat 16'. His ambition has transformed him into one of the most dangerous figures in the danger-laden world of covert operations.

Simmons confronts Jason Wynn at a lavish reception, furious about the betrayal and the havoc it's caused. In the heat of the confrontation, he kills Jessica Priest and escapes, discovering the power of Spawn's mystical armor.

The Violator convinces Jason Wynn to implement a device that will unleash Heat 16 globally if Wynn's heart stops. This sinister scheme aims to lure Simmons into a deadly confrontation with dire consequences.

Tensions rise as The Violator battles Spawn, but Simmons is saved by Cogliostro, another Hellspawn. Cogliostro begins instructing Simmons in the use of his necroplasm armor, preparing him for the battles ahead.

As Terry learns about Wynn's corrupt activities from a crucial text, he becomes ensnared in a deadly trap set by Jason Wynn. The situation escalates when Spawn arrives to intervene, leading to a tense standoff.
