Thirteen years after the original events, two suicides spark an investigation by Detective Koiso and his partner into a string of mysterious deaths. Each victim dies after a video plays on a device, and a whispering voice says You're not the one just before the end. The case points toward a cursed creation by the online artist Kashiwada.

Akane Ayukawa is a compassionate homeroom teacher. The film reveals she is telekinetic and once used her powers to protect her high school from a violent attacker, a past that earned her the label freak. Her power becomes crucial as the mystery unfolds.

Risa, the schoolgirl's best friend, begins digging into the cursed video before it vanishes from the internet. The trail leaves behind an enigmatic Error 404 message, a clue that triggers playback when a viewer is alone. These elements set the online hunt as a key thread of the case.

Within the video, Kashiwada appears to allow himself to be killed by a long-haired woman, a haunting tableau that foreshadows the danger to come. A spirit warns Akane that she is the one, sending ripples through their world. The video ends by forcing a doorway between the screen and real life.

Detective Koiso and his partner uncover Kashiwada's apartment, a curated and nearly artificial space. The wallpaper and furnishings form a mosaic that hides notes and the killer's history in white butterfly patterns. The landlady notes that the place feels superficial, deepening the sense of manufactured reality surrounding the case.

Akane and Koiso trace clues to the Yamamura household beside the old inn and near a derelict mall by the well. A grotesque Sadako facsimile materializes and attacks Koiso, while the women thrown into the well reappear as imperfect versions of Sadako who swarm Akane. The eerie confrontation tests Akane's resolve and powers.

The video’s pull becomes personal when it targets Akane and it homes in on their home, and multiple screens flash the ghostly woman. A towering LCD display truck on the street projects a giant image of the specter that abruptly snatches Takanori away. The escalating haunt drives the pursuit toward the truth behind the curse.

On the Yamamura site, Akane and Koiso confront the swarm of copies and the lingering Sadako menace. The imperfect versions threaten to overwhelm them, but Akane fights back with her telekinetic power. The showdown marks a turning point in breaking the curse's grip on the living.

Amid the ruins, Akane locates Takanori trapped inside a mobile phone at the center of the wreckage. She battles the imperfect Sadako copies, and fear-based action triggers a decisive blast that decimates the clinging specters. The power shows how her abilities can alter the fabric of the haunting.

The final battle moves to the roof where the real Sadako awaits. Sadako claims that they are the same, but Akane denies the accusation, arguing that she fights to protect people. In a self-sacrificing act, Akane trades her life for Takanori as Sadako merges with her, hair wrapping around and burying her.

Takanori escapes when he destroys the phone, but the roof collapses and Akane reappears on the floor, alive but covered in Sadako's hair. The ambivalent ending leaves the fate of the world still entangled with the artificial surroundings.

In the mid-credits scene, Kashiwada's landlady moves away while whispering that everything might be artificial. The line reinforces the theme that the world may be constructed or manipulated by unseen forces.

In the post-credits, the video intro replays with the chilling new line here we go again. The sequence hints that the cycle could recur within a similarly constructed reality.
