In 1935 Florida, Nicky Rocco is betrayed and executed in the swampy backwoods while his pregnant lover Ruby Claire watches. He dies with a vow of vengeance on his lips. As he dies, Ruby goes into labor and gives birth to their daughter, Leslie Claire.

Leslie Claire is born; Ruby raises a mob-connected life

Following Leslie's birth, Ruby's life is shadowed by the mob. She raises her daughter with the help of a lover and henchman, Vince, and keeps Jake Miller, a blind ex-mobster, in the household.

In 1951, Ruby has become the operator of a drive-in theater near her home, employing former mobsters to run it. Her 16-year-old daughter Leslie is mute, living with Ruby, her lover Vince, and Jake Miller, the blinded ex-mobster.

During a drive-in screening, projectionist Jess Littinger experiences poltergeist activity in the projection booth. He is found hanged, and Ruby dismisses it as suicide while ordering Vince to hide the body.

Lila June is given a stolen necklace from Louie at the concession stand. Louie tries to rape Lila, but she escapes. The next morning, Louie is found dead, hanged in the woods, a crime Ruby fears reflects Nicky's return.

That same night, Vince and Ruby carry Louie's corpse to the swamp to dispose of it. They fear they are dealing with something more than a simple killer and suspect a supernatural force is at work.

Jake Miller appears in his wheelchair and is stabbed to death. Later, Ruby reveals a jar containing Jake's preserved eyeballs, proving her dedication to Nicky and cementing the mob ties behind the hauntings.

Vince is urged to flee by Dr. Keller and encounters Nicky's apparition. Ruby disappears from the house, and Vince and Keller flee into the swamp, where they witness Ruby walking hand-in-hand with Nicky toward the water. She screams as she is dragged underwater to her death by Nicky's skeleton.
