Shannon, Rachelle Lefevre and her husband Steven, Ryan Phillippe, face a life-altering car accident that leaves them unable to build a family of their own. Driven by a longing for parenthood, they pursue adoption through the International Rescue Adoption (IRA), a Puerto Rico–based organization run by Gabrielle Reigert, Jacki Weaver. After paying a steep $60,000, the couple relocates to a resort while they await Nina’s United States passport, hoping their new chapter will begin soon. The peace of their plan is short-lived, as Steven grows wary of the neighbors next door—Benjamin, John Cusack; Salo, Jandres Burgos; and Paola, Veronica Foo—especially after a tense bar encounter during which Benjamin’s offhand talk about Nina sparks a heated confrontation and sets a troubling tone for what lies ahead.

Two days after the adoption, Nina vanishes into the island night, plunging the new family into a nightmare. They turn to a police officer, Luis Guzmán, who bluntly explains that the IRA’s adoption paperwork is not properly recorded and that IRA’s Old San Juan headquarters stands deserted. The officer’s suspicion grows that Reigert’s operation is a ruthless confidence scheme: it promises vulnerable couples an orphan and then disappears with their money in a matter of days. The revelation hits hard when Steven admits that Reigert had demanded an additional $40,000, a detail he kept from Shannon, fueling questions about what they were really stepping into. As fear tightens its grip, the couple realizes they’ve stumbled into a dangerous trafficking scheme that preys on dreamers hoping for a family.

That night, Benjamin and his aides kidnap the couple and reveal themselves as the very traffickers Reigert works with. Nina is held hostage as the trio coerces Shannon and Steven to hand over every last cent they have, money that largely embodies compensation for the car crash that deprived them of a child. The next day, Steven is forced at gunpoint to go to the bank to withdraw the savings. He notices that Benjamin’s mansion is in Maunabo and, with a reckless resolve, returns there with the money obtained from Salo. Meanwhile, Shannon manages to break free, seizing Nina and distracting Paola long enough for Steven to seize Paola’s gun and drive away with his wife and daughter in tow. They’re pursued by Benjamin and Salo, and the chase ends tragically for Salo, who is killed when a truck crashes into his car.

After a harrowing escape, Benjamin crashes his own vehicle and is presumed dead, but the trio’s peril is far from over. They’re briefly escorted back by the officer, only for Benjamin to reappear, killing the officer and dragging them to Reigert’s villa. A bitter quarrel over their spoils erupts, culminating in Benjamin shooting Reigert dead. Steven regains his footing, follows Benjamin, and catches up to him as he chases Shannon into a dilapidated, shadow-filled building. In a crucial confrontation, Steven arrives just in time and kills Benjamin, sparing Shannon from a lethal fate. But the danger isn’t over—Nina arrives, visibly shaken, and seizes Benjamin’s gun, pointing it at Steven and Shannon. With steady resolve, they manage to calm her and coax her to put the weapon down, choosing to stay together as a family instead of letting tragedy tear them apart.
