The story opens with André Toulon's grave being unearthed at the Shady Oaks cemetery, located behind the Bodega Bay Inn. As the scene unfolds, Jari Virman as the Man slug, observes Pinhead dusting off and opening Toulon's casket. In a haunting display, Pinhead climbs out and applies a vial of glowing neon liquid onto Toulon’s skeleton, instigating the resurrection of the puppet master. As the skeleton’s arms rise into the air, Puppet characters like Tunneler, Leech Woman, Blade, and Jester watch silently from the edge of the grave, witnessing this dark ritual.
Several months later, a team of researchers, including parapsychologists Carolyn Bramwell, her brother Patrick, as well as the charming Lance and Wanda, arrive at the hotel to investigate the mysterious murder of Megan Gallagher. This victim had her brain forcibly removed through her nose by Blade, one of Toulon’s deadly puppet creations.
The investigation becomes more complicated as they suspect Alex Whitaker—who, overwhelmed by the traumatic events of the initial film—has become mentally unstable and is confined to an asylum. His disturbing seizures and premonitions are dismissed as hallucinations by the staff, although they hint at deeper and darker messages.
Meanwhile, a psychic named Camille Kenney attempts to warn the group about imminent danger after spotting two puppets lurking in her room. Her warnings go unheeded, but shortly after, she is attacked and kidnapped by Pinhead and Jester. When morning comes, Camille's belongings and her car are found at the hotel, but there is no sign of her. Carolyn gets in contact with Camille’s son to update him on her disappearance.
Later that night, tragedy strikes. While Patrick sleeps, Tunneler drills into his head, leading to his gruesome death. Lance and Wanda rush into the room to find the aftermath, and in fury, Lance crushes Tunneler with a lamp to end the puppet’s rampage. The surviving investigators dissect the puppet and uncover a shocking secret: these puppets are not remotely controlled. Instead, they operate on a special chemical that supplies an artificial intelligence, fueling their actions and autonomy.
The next day, a mysterious man named Eriquee Chaneé arrives at the hotel, claiming to have inherited the property after Megan’s death. He states that he's just returned from Bucharest. His appearance raises suspicions among the group, but he allows them to stay and investigate, warning them that his quarters are off-limits. Soon after, Michael arrives, worried about his mother Camille.
That evening, Blade and Leech Woman sneak into a nearby farmer’s house, where Leech Woman kills the husband, Matthew, and takes his eye. The farmer’s wife, Martha, fights back fiercely, throwing Leech Woman into her furnace. As she tries to shoot Blade with a shotgun, a new puppet, Torch, appears, killing Martha with a built-in flame-thrower. Blade then takes a charred piece of Martha’s remains and, along with Torch, returns to Toulon’s quarters. It’s revealed that Toulon believes Carolyn is a reincarnation of his late wife, Elsa. Flashbacks transport us to Cairo in 1912, where Toulon and Elsa first obtained the formula for animating inanimate objects, hinting at their deep connection and Toulon’s ongoing obsession.
The following day, Michael and Carolyn venture into town to learn more about Eriquee Chaneé and locate Camille, who they believe might be in danger. Meanwhile, the puppets are behind a series of killings—they weaken and require the brain tissue of living beings to sustain their life force. Carolyn finds no official records linking Chaneé to the disappearances, but she begins to suspect that he’s connected to her brother Patrick’s death and Camille’s disappearance.
The romantic tension escalates as Carolyn and Michael spend an intimate evening together, as do Lance and Wanda. However, the puppets, driven by despair and hunger, brutally murder Lance and Wanda, using their tissue to strengthen Toulon’s formula. Carolyn eventually sneaks into Toulon’s room, discovering two life-sized mannequins hidden away. Toulon, mistaking Carolyn for Elsa, ties her up. Michael rushes to her rescue, battling Torch, Pinhead, and Blade along the way. During the chaos, a dumbwaiter opens to reveal Jester and the ghostly apparition of Camille. Toulon secretly transfers his consciousness into one of the mannequins, revealing that he intends for the group to live with him forever.
Horrified, the puppets realize they have been manipulated for Toulon’s evil purposes and begin to torment him. Michael intervenes, rescuing Carolyn, and they flee from the hotel. In the attic, Torch sets Toulon ablaze, leading to his fall from a window to his death. As Toulon perishes, Jester returns Camille’s soul into a female mannequin, hinting at a possible continuation of Toulon’s dark legacy.
The story concludes with Camille, now revived as a living mannequin, taking the puppets in her car. She intends to drive them to the Bouldeston Institution for the Mentally Troubled Tots and Teens so they can "enchant" and influence children with their eerie presence, leaving the audience with a chilling sense of what’s to come.
