In Las Vegas, Tony exits a casino and delivers a voiceover about how he views life and loyalty. When he starts the car, it explodes, instantly cutting to a dark, violent mood that frames the film's present. The moment sets up the ongoing tension between surface glitz and deadly undercurrents.

The film introduces Vincenzo Armani Windbreaker Cortino, the clumsy son of a Sicilian postman, born in Italy. His humble beginnings foreshadow a destiny shaped by crime and clan loyalty. These flashbacks lay the groundwork for a family saga that will span generations.

While Vincenzo delivers for his father, he trips and the parcel bursts open, revealing a mysterious white powder. The recipient concludes that the delivery boy has seen too much and tracks Vincenzo to a street fair, where he kills Vincenzo's father. The boy escapes with his life, setting the stage for a lifetime of violence and exile.

The boy escapes to America, where he grows to manhood, marries, and struggles with poverty. This period lays the foundation for his eventual rise to power within the mafia, culminating in his ascent to boss.

Tony returns from the Korean War with his idealistic Protestant girlfriend Diane, bringing her to his brother Joey's wedding reception to be introduced to the family. The scene parodies Connie Corleone's wedding from The Godfather, signaling the film's affectionate homage. Tension rises as Tony tries to bridge two worlds: his love and his Sicilian roots.

During the festive gathering, Vincenzo endures a brutal attack: 47 bullets strike the scene but he survives. Tony vows to kill Gorgoni, a drug lord Vincenzo had refused to do business with before the assault. Diane leaves Tony, accusing him of abandoning the peaceful ideals of his youth.

Tony avenges the attack and goes into hiding in Las Vegas, where Cesar Marzoni offers him a chance to manage his casino, The Peppermill. Tony accepts, and his casino enterprise proves highly successful. Pepper Gianini, hired by Marzoni, is introduced as part of a plan to distract him and drive a wedge between him and Joey.

Vincenzo recovers from his 47 gunshot wounds and visits Las Vegas, where he officially names Tony his successor. Joey, furious at being passed over, is told 'You get Wisconsin' as his new assignment, signaling his diminished status.

The Don returns home, only to be assassinated by his five-year-old grandson, Chucky, who sprays him with malathion in a parody of a famous mob moment. The act seals Tony's path to vengeance and power within the family.

The present thread intensifies as Tony catches Joey and Pepper cavorting in a hotel room. He walks out in disgust, but his car explodes soon after, leaving him horribly but temporarily disfigured.

Tony attends his father's funeral in a wheelchair, spotting the killers when he sees little Chucky taking a payoff from a rival Don. He decides to postpone his vengeance until he can win back Diane's love and put his life in order.

Diane has by this time become President of the United States and is on the brink of declaring total world disarmament. Tony goes looking for her and persuades her to put world peace on hold until after their wedding.

During the ceremony, with help from Vincenzo's Mom Sophia, Tony's right-hand man Nick Molinaro, and a generic henchman, he settles the family's accounts in an orgy of slaughter reminiscent of The Godfather. The finale includes the harpooning of Barney the Dinosaur as a bizarre bonus.
