The narrative begins with a festive birthday celebration hosted by Verna Sawyer for her youngest son, Jedidiah. It marks a significant moment as Jed is introduced to the family's brutal traditions during this event.

As part of his initiation, Jed receives a chainsaw and is coerced to inflict pain on a thief caught stealing livestock. However, he is disturbed by the act and refuses, leading to a gruesome end for the thief at the hands of Grandpa.

A few months later, young couple Betty Hartman and Ted Hardesty find themselves driving through the Sawyer territory. Their encounter with Jed, who appears injured, leads them into a trap set by the sinister Sawyer family.

While Betty is lured into a rundown barn, she experiences a shocking ambush by the Sawyer clan. Her father, Sheriff Hartman, eventually discovers her lifeless body, leading him to seek vengeance against the family.

In response to Betty’s death, Sheriff Hartman takes young Jed into custody. He is sent to a mental asylum for troubled youths, marking the beginning of a dark chapter in his life as he grapples with the family's legacy.

Fast forward to 1965, and we meet Lizzy, a new nurse at the asylum. She quickly forms an unexpected bond with two psychiatric patients, Bud and Jackson, unaware of the turmoil that lies ahead.

Verna Sawyer arrives at the asylum with a court order to free Jed. When her request is denied, she resorts to violence, instigating a riot that leads to numerous deaths among staff and patients.

During the ensuing chaos, Lizzy and Jackson are taken hostage by two deranged escapees, Ike and Clarice. Their nightmare escalates when they stop at a diner, where a gruesome massacre unfolds.

Sheriff Hartman and Deputy Sorrell discover the bloody aftermath of the diner attack. During their investigation, they learn that the teenage Jed is among the surviving escapees, heightening the tension.

The fugitives attempt to hide in an abandoned mobile home, where a series of disturbing events take place. Tensions rise further when Ike is killed by Bud, further deepening the grim atmosphere.

The following day, Sheriff Hartman captures Clarice, who defiantly taunts him before facing a tragic end. This escalating brutality serves to emphasize the high stakes involved in the ongoing conflict.

As chaos ensues, Lizzy and Jackson suffer a loss when Bud is inadvertently killed. Fueled by rage and desperation, they steal a police car in a bid to escape Hartman's relentless pursuit.

In a dramatic twist, Lizzy communicates with Officer Sorrell, who reveals the shocking truth about Hartman having Jed at the site of Betty's murder. This revelation sets the stage for a tense family showdown.

As Verna seeks to rescue her son Jed, the film explodes into a climactic confrontation between her and Sheriff Hartman. The revelations surrounding Jed's identity create a twist that brings the family's legacy full circle.

In the final moments, Verna stitches up Jed’s injuries while initiating him into their gruesome family rituals. The chilling scene concludes with Jed creating a mask from Lizzy’s face, marking his descent into the infamous Leatherface legacy.
