Kamen Rider Ex-Aid the Movie: True Ending

Days after the previous events, a new Bugster Virus strain spreads among the population. A group of Ninja Players led by Fuma invades Seito University Hospital. Emu, Poppy, Hiiro and Taiga transform into Kamen Riders to stop them, but are defeated; everyone except Emu falls into a coma.

During the invasion, Fuma uses his Gashacon Bugvisor on Madoka Hoshi, rendering her unconscious. The attack demonstrates that this new crisis is more than a single battle, threatening vulnerable patients.

Madoka wakes up to a startling contrast as Poppy is surprised to see her friends at a School Festival, as if nothing had happened. The normal scene hints at the VR world’s influence and deepens the mystery around Madoka.

Kiriya and Kuroto investigate the origin of the new virus and discover that Masamune had negotiated with Johnny Maxima of Machina Vision regarding Kamen Rider Chronicle. The deal never closed, and important Rider Gashat and Gamer Driver data were stolen along with company data. The finding links the crisis to Machina Vision.

Kiriya confronts Maxima and Fuma, who is revealed as Kagenari Nagumo, Madoka’s father. Nagumo explains that he sent Madoka and others into the VR world for her sake and repeats the plan with Maxima and Kiriya. The revelation frames the VR world as a trap that could decide their fates.

Kuroto instructs Emu to enter the VR world to rescue them and gives him a new Gashat called Mighty Creator VRX. Emu prepares for the leap into the virtual world, aware that the rescue depends on his new powers.

Inside the VR world, Kagenari chases Emu, but is defeated by Emu using his growing powers. The battle in virtual space showcases Emu’s evolving abilities and sets the stage for a potential escape.

Emu creates an exit for everyone to escape the VR world, but Madoka refuses to leave and stays behind. Maxima appears and uses the power she claimed for himself, complicating the rescue and signaling a new threat.

Back in the real world, the Riders confront Kagenari about his actions and learn that his goal was to let Madoka live happily in the VR world, a choice born from fear for her survival. The moral weight of his decision weighs on the team.

Maxima reveals that he has trace data of Gamedeus and infects himself to become Gamedeus Machina, planning to destroy the world. The Riders are overwhelmed as his power rises, signaling an existential threat.

Kuroto and Parad sacrifice themselves to restrain Gamedeus’ powers, buying the others time. Maxima flees after being restrained, leaving the world on tenterhooks as the final struggle approaches.

Back at the recovery room, Hiiro, Emu and Asuna perform an emergency operation to save Madoka. Meanwhile Taiga and Kiriya mount an assault on Machina Vision to confront the threat head-on.

Emu reveals to Kagenari that Madoka’s true wish was his happiness, and that he must be by her side for her consciousness to return. Maxima launches a final attack, but Emu transforms into Hyper Muteki to destroy him. Madoka awakens beside her father as the crisis ends.

With the Bugster threat eradicated, the Riders return to their daily lives. In a post-credits scene, Emu and Parad confront Bugsters and are intercepted by Kamen Rider Build, who defeats Parad and extracts Ex-Aid’s essence from Emu before departing.
