Mary Lou died during the 1957 high school prom when Billy Nordham threw a stink bomb at her in retaliation for discovering she was using him for popularity. The bomb accidentally ignited her dress, causing her tragic death in front of the entire school.

Earlier in the prom, Billy discovers Mary Lou with Buddy Cooper, establishing the betrayal that motivates his lethal prank.

Vicki Carpenter accidentally unleashed Mary Lou's vengeful spirit when she opened an old trunk in the school's prop room containing Mary Lou's crown and cape. She left the items overnight, which allowed Mary Lou's spirit to resurrect and begin her reign of terror.

The trunk had been forgotten in the school basement for 30 years, waiting to be opened.

Mary Lou possessed Vicki as a vessel to carry out her revenge against those she blamed for her death. During detention, Mary Lou literally pulled her spirit through a blackboard that transformed into a whirlpool, giving her physical form to continue her killing spree.

Vicki had been experiencing disturbing visions of Mary Lou ever since opening the trunk, signaling the spirit's growing influence.

Vicki Carpenter killed Buddy Cooper while under Mary Lou's possession. She confronted him at the church and murdered him before anyone could intervene, fulfilling part of Mary Lou's vendetta against those connected to her death.

Buddy was the student Mary Lou was found with in 1957, making him one of her primary targets.

Jess became Mary Lou's first victim when she attempted to steal jewels from Mary Lou's crown. Her death was later wrongfully ruled a suicide attributed to an unwanted pregnancy, masking the supernatural nature of her murder.

Vicki found the crown and cape in the trunk, and Jess was curious about the valuable-looking jewels.

Mary Lou's vengeful spirit returned because she died a tragic, violent death at a young age, betrayed by Billy who caused the fire. She was also betrayed by Buddy who was with her, and her death was fueled by humiliation and rejection, leaving her spirit seeking justice against everyone connected to that night.
