Crystal 'White Girl' Van Meter, a 15-year-old prostitute, is sentenced to 25 years for a long list of crimes. She is transferred to a minimum-security hospital to seek treatment for bulimia. The sentence confines her and sets the stage for her unlikely partnership with Cyclona.

White Girl teams up with Angela 'Cyclona' Garcia, a teenage serial killer, and they escape from the hospital. During the escape, White Girl injures herself on a barbed-wire fence. The two fugitives vanish into the night.

Cyclona believes Sister Gomez, a Catholic/spiritualist healer, can help White Girl with her bulimia, so they head toward Tijuana. Their plan hinges on finding Sister Gomez and gaining her aid. The trip marks the first step in their flight from confinement and trauma.

On the run, Cyclona murders a family and engages in disturbing acts involving the corpses. The violence escalates quickly as they push deeper into their road trip. This brutal event foreshadows the darkness of the path they are on.

White Girl grows wary of Cyclona's lack of medication and insists on occasional doses. They steal the family's car and continue south toward Mexico, abandoning any remaining sense of safety. The escape becomes a ruthless drive toward an uncertain future.

Cyclona reveals that Sister Gomez once saved her from abuse and possibly other threats; they discuss the past as they travel. These revelations deepen their bond and justify their trust in seeking her help. The road trip takes on a more personal, haunted purpose.

They hop a freight train to continue moving south; along the way they are assaulted by a transient crack addict. Cyclona kills the attacker to protect White Girl and the two make off with his bag of crack and guns.

Lost and confused, they leave a trail of crack that shady men with crow feathers on their hats begin to pick away. They reach the border and have a stand-off with two customs officials, whom Cyclona kills. White Girl, angry at Cyclona's killings, pistol-whips Cyclona and they race toward the suburbs of Tijuana.

In Tijuana, White Girl uses her wits to make money and survive while Cyclona remains her partner in crime. They check into a run-down motel to rest and plan their next move. Their relationship grows more intense as they rely on each other to endure the dangerous world they've entered.

They spot a poster showing Sister Gomez is in town, and Cyclona urges visiting her. They visit Sister Gomez at a gaudy mission house that serves as her front, and she greets them with a feast. The moment marks the start of a deeper, dangerous entanglement with the healer.

Cyclona disappears as White Girl is forced to work for Sister Gomez in exchange for food. White Girl discovers a basement where children are housed in a disturbing setup, revealing the true horror of the mission. Cyclona is found bound in a bondage contraption, and Sister Gomez brags about her control and her chilling plans for White Girl.

White Girl frees Cyclona and defeats the cult members. Sister Gomez is revealed to be a trans woman. She is killed in a fiery end as the cult's hold collapses, with her screams echoing the terror of her long reign.

White Girl, keeping a promise she made earlier, kills Cyclona in a tearful moment rather than let her be captured. She makes a deal with the federales and leaves with her lawyer/pimp, declaring, 'I'm not hungry anymore.'
