Twenty years after their last adventure, Harry Dunne visits his long-time friend Lloyd Christmas at a mental institution. During this visit, Harry realizes that Lloyd's feelings for Mary Swanson were just a prank, prompting him to reconsider their friendship and the life they once shared.

In a bid to escape their boring lives, Lloyd convinces Harry to break out of the mental institution together. They decide to return to their old apartment, igniting a spark of adventure and nostalgia.

Upon returning to their apartment, Harry reveals a disturbing truth: he has a kidney problem and urgently requires a transplant. This revelation marks the beginning of their quest to find a suitable donor for Harry.

Harry's quest takes a turn when he receives a postcard from his former girlfriend, Fraida Felcher, announcing that she is pregnant. They believe that Harry's potential child could be the kidney donor he desperately needs.

Lloyd and Harry visit Fraida at her workplace, a funeral parlor, to gather more information. Fraida reveals that she gave up her daughter for adoption, leading them to a photo of the girl, named Fanny, which captivates Lloyd.

Motivated by his attraction to Fanny, now called Penny, Lloyd persuades Harry to 'borrow' a hearse and journey to Maryland in search of her. Their unconventional means of travel sets the tone for the absurd adventures to come.

At the convention, Penny's adoptive mother, Adele, plans to poison her husband with the assistance of the housekeeper, Travis. This treachery becomes known as Harry and Lloyd dig deeper into the family dynamics.

Adele uses the opportunity to send Harry and Lloyd on a mission to retrieve Penny's package, claiming it holds immense value. This sends the duo on yet another chaotic adventure filled with misunderstandings.

As Harry and Lloyd navigate the convention, mistaken identities and comedic mishaps ensue. Harry realizes Lloyd's feelings for Penny, creating tension between the two friends during the escalating chaos.

In a shocking twist, it is revealed that neither Harry nor Lloyd is Penny's father. Instead, her biological father is actually a deceased friend from high school, causing confusion and hilarity.
