In the heart of Kathmandu, Nepal, a formidable sorcerer named Kaecilius played by Mads Mikkelsen orchestrates a shocking attack on the hidden sanctuary of Kamar-Taj. He and his followers brutally eliminate the librarian and steal a crucial ritual from the sacred texts belonging to the Ancient One, a timeless sorcerer who has imparted her wisdom to many, including Kaecilius himself. In pursuit of these traitors, the Ancient One captures Kaecilius in a mirror dimension, though he manages to evade her grasp and escapes with the stolen knowledge by creating a portal.
Meanwhile, renowned neurosurgeon Stephen Strange, portrayed by Benedict Cumberbatch, faces a devastating blow as an unfortunate car accident leaves him with severely damaged hands. Once celebrated for his exceptional skills and steady grasp—skills honed through years of rigorous practice—he is now relegated to a life of frustration. Following a grueling surgery that involved 11 pins and extensive nerve damage, Stephen's recuperation journey is riddled with challenges and pain.
His former lover and fellow surgeon, Christine Palmer, played by Rachel McAdams, attempts to support him through this bleak chapter. However, Stephen remains stubbornly optimistic about regaining the use of his hands and is willing to exhaust every option available, leading him to numerous experimental surgeries that ultimately yield no positive results. Facing rejection from the best medical minds, his determination only strengthens.
Strange's pursuit of hope leads him to discover the story of Jonathan Pangborn, a man who overcame paralysis under mysterious circumstances. Despite previous reservations about his career, Strange seeks Pangborn and learns about the extraordinary healing he experienced after encountering his guru in Kamar-Taj. He is inspired to venture there, but his journey takes many weeks as he grapples with skepticism about the mystical healing arts.
Upon reaching Kamar-Taj, Strange is taken in by Mordo, portrayed by Chiwetel Ejiofor, who introduces him to the Ancient One, played by Tilda Swinton. During his initial interactions, Strange is dismissive of her teachings until she reveals her astounding magical prowess, exposing him to dimensions beyond his comprehension. She likens Strange's arrogance to a filled glass, suggesting he must first empty it to learn.

Wong recounts the ominous threat of Dormammu, a powerful entity from the Dark Dimension, who craves dominion over Earth. The stolen ritual is intended to summon Dormammu, igniting a race against time as Kaecilius and his followers begin their perilous rituals.
As months pass, Strange's skills flourish, but he is warned by Mordo and Wong against breaking cosmic laws, as his hunger for power mirrors that of Kaecilius, who seeks eternal life in the face of sorrow. The ensuing chaos unfurls when Kaecilius manages to breach the London Sanctum, which leads to a dramatic confrontation involving Strange, Mordo, and the Ancient One. Tragedy strikes when Kaecilius reveals the Ancient One’s controversial source of power before fatally wounding her.
Heartbroken and driven, Strange takes it upon himself to save what remains. He employs the Eye of Agamotto to rewind time, ultimately confronting Dormammu in a battle that traps them in an infinite loop. Realizing he cannot defeat Strange, Dormammu makes a reluctant deal to leave Earth, alongside Kaecilius and his followers. Meanwhile, Mordo becomes disillusioned with both Strange and the Ancient One's disregard for natural order, deciding to sever their ties.
Strange returns the Eye, identified as an Infinity Stone by Wong, to Kamar-Taj, and chooses to reside within the New York Sanctum as he continues his mystical training. His journey culminates in a mid-credits scene where he decides to join forces with Thor, ushering in new adventures. Lastly, in the post-credits scene, Mordo confronts Pangborn to reclaim the energy he uses to walk, heralding a new chapter of conflict with the unwavering conviction that there are far too many sorcerers on Earth.
