Faith Chandler drops off John Davis at Shady Palms Clinic. Edith Mortley smothers the patient to end his life as part of the Mortleys' scheme, and Gordon Mortley buries the body on the clinic grounds. The fake treatment narrative is already in motion as they seek to bill the state.

When Faith returns to check on John, Gordon digs up the buried body. He crudely puppeteers the corpse to look as if the patient is still alive. The ruse is designed to maintain the illusion for Faith and keep billing the state.

Faith Chandler delivers Charles Bedowski, a man with a heart condition, to Shady Palms. The new patient becomes part of the Mortleys' larger deception to extract payment under the guise of treatment. The clinic's façade tightens as more patients arrive.

Edith and Gordon attempt to replace Bedowski's dying heart with a dead dog's heart. During the procedure, the Mortleys' cat grabs the heart, sabotaging the operation. The transplant fails, exposing the brutality behind their 'treatment'.

Gordon buries Bedowski's remains after the failed transplant. Edith then feeds pieces of the deceased man to the rats that inhabit the clinic's garage, further tainting the facility's already grim reputation. The events escalate the investigators' suspicion of the clinic.

An environmental health officer, Mr. Smith, arrives and warns that Shady Palms could be shut down for vermin and mismanagement. Edith responds violently by stabbing him, eliminating the immediate threat and heightening the clinic's peril. The murderous cover-up deepens as authorities close in.

Faith Chandler checks into Shady Palms and grows suspicious of the facility. Edith lures or confronts Faith and, after feeding her a rat-based meal she had prepared, stabs her, cementing the pattern of the Mortleys' killings. Gordon's stated fondness for Faith contrasts with the aggression of Edith's actions.

Louise Kagel, the clinic's alcoholic resident, witnesses Faith's murder. Edith then uses a syringe to kill Louise, removing another potential witness and clearing the way for the pair's crimes. The murder adds to the mounting body count in the garage and clinic.

Gordon and Edith place the bodies of Smith, Faith, and Louise in the clinic's garage. Their stench and presence draw the notice of outsiders, intensifying the investigators' interest in the facility. The garage becomes a gruesome repository for the Mortleys' victims.

A police lieutenant who had stopped by to visit Bedowski opens the garage and discovers the grisly contents. The discovery confirms the Mortleys' long-running scheme and ends in emotional collapse for Edith and Gordon. The film then reaches its somber conclusion as the couple sits together in defeat.

The revelation at the garage caps the Mortleys' crime spree. Edith and Gordon share a dejected moment, watching the consequences of their scheme unfold as the narrative ends.
