In Ohio, Lucy Vincent and her three children are found brutally murdered after Lucy's birthday party. Ed Vincent is missing and presumed the killer. Veteran homicide detective Joe Steiner is assigned to the case, but with Ed missing the investigation goes cold.

Two men are involved in a fiery wreck that kills one and leaves the other disfigured. It is discovered that one of them is Allen Devlin, but identification is impossible: the deceased's body is burned and the survivor has amnesia.

Over the next year, Allen undergoes facial reconstructive surgeries and, with nurse Chris Graham, battles to remember his past. As memories surface, the two grow closer and fall in love.

Six years later, Allen and Chris are happily married with children. Allen is a successful realtor while Chris stays at home to raise their kids.

Steiner, now forced into early retirement, still pursues clues. A newspaper article featuring the Devlins triggers a tip that Allen bears a strong resemblance to Ed Vincent.

Steiner travels to California to investigate and encounters Allen, openly voicing his suspicion that Allen may be Ed Vincent.

Police Chief Michael Patterson urges Steiner to drop the investigation. Steiner contends that Allen's behavior hints at a split personality, with Vincent as a subconscious aspect of Allen.

Allen hires private investigator John Davey to verify Steiner's claim. Davey tracks down Theo Grant, who lent the vehicle from the crash; Grant refuses to talk without compensation.

That night, Chris receives a call from a man with a distorted voice, claiming Lucy and accusing her of an affair. Later, a woman is attacked in a park by a leather-masked attacker, linking back to the Ohio killings.

Davey is found murdered, and Patterson orders a search of the Devlin home. The search yields nothing that corroborates Steiner's suspicions.

Another attack occurs; Chris discovers a black leather mask. At the advice of his lawyer, Allen admits himself to a psychiatric hospital.

At the Devlin home, Chris confronts Allen as he reveals himself to be Ed Vincent, confirming Allen is the killer behind the Vincent murders and that the wreck victim was the real Allen Devlin. Patterson is found dead during the confrontation, and Steiner arrives just in time to shoot Allen and save Chris. Days later, Chris visits Steiner to thank him.
