In Spain, 1982, Marisa brings her pregnant daughter Lucia to Madrid seeking help. Lucia finds herself at Peñagrande, a reformatory housing young women facing unplanned pregnancies. While there, she develops close bonds with the other residents and uncovers a disturbing reality: her baby is to be taken from her against her will.

What Happens at the End of Birth?

In the film’s final act, Celie’s desperation to restore her six‑year‑old daughter drives her to a dark partnership with the aloof pathologist Rose. Rose has been perfecting a regeneration serum made from fetal fluid, a recipe she first derived from a fetus she kept in a jar after an illicit self‑impregnation. When Lila’s body is placed on a ventilator in Rose’s apartment, the serum keeps her heart beating and she begins to recover, but the supply of fluid soon runs out.
Because Rose can no longer produce more serum—her reproductive system has been destroyed by infection—the two women turn to the hospital’s records and locate Emily Parker, a woman whose pregnancy has been long‑desired but who has no other children. Celie breaks into Emily’s home, drugs her tea, and triggers a seizure that lands Emily in the emergency room. The medical team prepares for a premature delivery, assuring Emily that the baby will be saved. Emily herself collapses and dies, but the doctors extract the placenta and the amniotic fluid they need.

Back at Rose’s makeshift lab, the fresh fetal material is mixed into the serum and administered to Lila once more. This time the reaction is definitive: Lila’s pulse steadies, she draws a breath, and the film ends with her lying in Rose’s bed, alive again. The closing image suggests that the cycle may repeat—whenever the serum is exhausted, the mothers will resort to equally unethical means to keep Lila breathing.
Thus the ending ties together the film’s two opposing worlds—Rose’s clinical obsession with regeneration and Celie’s maternal drive to protect her child—showing how far each will go when death is confronted, and leaving the audience to wonder whether the reborn Lila will ever truly be the daughter they both lost.
