Basket Case 3: The Progeny

Duane Bradley is separated from his monstrous former conjoined twin Belial and locked in a straitjacket in Granny Ruth's haven for 'unique' (deformed) individuals. After several months of captivity, Granny Ruth plans a road trip to Doctor Hal Rockwell's home, hoping he can help in giving birth to Belial's girlfriend Eve's babies. The warning about Belial's danger underscores the fragile bond between the twins.

Granny Ruth sternly warns Duane to stay away from Belial, who has stopped speaking to him telepathically since their failed attempt to rejoin. She announces a road trip with the group to Hal Rockwell's house, where Eve's babies will be born. The warning sets a tense tone as the journey begins.

The group travels by bus toward Hal Rockwell's home and stops at a drug store along the way. Granny Ruth meets Sheriff Andrew Griffin, while Duane, attempting to wriggle out of the bus window, encounters the sheriff's daughter Opal. The stop foreshadows the complications that will come from trying to escape and from Duane's interest in Opal.

Granny Ruth sweet-talks Sheriff Griffin into letting the bus depart, despite the vehicle being illegally parked. Her maneuver buys them time but heightens the risk of pursuit as they continue toward Hal Rockwell's house. The road trip resumes with tensions running high.

The party reaches Doctor Hal Rockwell's house in Peachtree County. Duane manages to escape through a window after Granny Ruth decides to give him a chance and frees him from his straitjacket. His escape triggers a sequence of events that places the family on a perilous path toward Eve's birth.

At Hal's home, Eve is about to give birth as Belial wears surgical attire and Hal experiences a flood of memories about their earlier separation. Belial is drugged to calm him, and Eve's birth proceeds under the observation of Little Hal. Eve gives birth to Belial's 12 children, while the household braces for the chaos to come.

Deputies Bailey and Baxter break into the Rockwell residence, drawn by the million-dollar reward for Duane and Belial. They discover the newborns and, mistaking Eve for Belial, shoot and kill her as the freaks react in shock and rage. The incident intensifies the race to protect the babies and settle old scores.

At the police station, Sheriff Griffin discovers his daughter Opal wearing dominatrix attire and attempting to seduce Duane, and he orders her to stand down. Bailey and Baxter arrive with the babies, prompting Griffin to investigate the Rockwell residence. Belial storms the station, and Opal is accidentally shot by Brody, who is then killed by Belial.

Belial attacks the police station; Duane is released by Opal and joins forces with Belial. Duane reconciles with Belial, and Little Hal constructs a robotic exoskeleton for him, arming Belial for revenge and to reclaim the babies. The unlikely alliance signals a shift toward a more violent confrontation.

Eve's funeral triggers Granny Ruth to rally the freaks and move toward the abandoned factory where Sheriff Griffin awaits the trade. Griffin lays out his deal: Belial for the babies, a bargain the freaks seem prepared to accept on their own terms. The crew prepares to move against the sheriff and secure the children.

At the abandoned factory, Belial and Sheriff Griffin clash, with Belial's armored exoskeleton giving him the edge. The sheriff ambushes and trips Belial, and Belial's flailing blows culminate in the babies swarming to kill Griffin when he falls into their cradle. The chaos leaves the fate of the babies in question as the freaks prepare for their next move.

Following the confrontation, the freaks storm the Renaldo talk show, where Belial mauls the host. Granny Ruth delivers a bold message to the television audience, declaring that the freaks will no longer hide and will fight back if oppressed. The film ends with the group embracing a new, defiant presence in the world.
