The film ends with Anna finally piecing together the grim truth of her existence. She discovers that the “husband” who has been protecting her, James, is not a partner at all but a military operative assigned to guard a single immune clone until she gives birth. Helen and her partner Peter have been living on a quarantined island where the government keeps its last‑ditch genetic experiment alive: a woman whose immune blood can produce a child that might survive the pandemic that has wiped out most of humanity.
When Anna manages to free herself from the laboratory’s restraints, she confronts a room full of identical clones—failed attempts that were burned when they became infected. Helen reveals that the program has already created her ninety‑six times, and each clone was meant to be a vessel for an immune baby that the military would then sell to the highest bidder. Despite this betrayal, Helen admits that Anna’s pregnancy is the first successful hope: both mother and unborn daughter are immune, offering a chance for a new human lineage.
Determined to escape the militarized control, Anna and Helen plan to seize the island’s boat and head north, to one of the few “safe islands” where the virus has not taken hold. Their attempt is interrupted when Dr. Henry, now in full military garb, arrives with troops, seizes the newborn, and orders Helen to erase Anna’s memories once again. In the ensuing chaos Helen kills several soldiers, and Anna, using a gun hidden in a medicine box, shoots the remaining guard. When Henry tries to leave with the baby, Helen turns the gun on him, ending his threat in a single shot.
The final scene shows Anna, cradling her daughter, sailing away with Helen toward the untouched northern islands. For the first time since she awoke on the cliff, Anna is free of the lie and the laboratory, able to live as a human rather than a manufactured specimen, and the three of them drift toward the possibility of a fresh start for the species.
