Shivashankar, a crippled multimillionaire, decides that his spoiled son Vishnu needs a reality check. He sends Vishnu and his friends to a rural village, framing it as a charitable mission. Vishnu is unhappy about the plan but is pulled into the assignment by his father’s pressure.

A local pizza restaurateur lures Vishnu and his friends with the claim that Thottapuram is a single large brothel. The deception is meant to scare them and ruin their mission. The lie seeds mischief as the group steps into the village.

Divya and her fellow students arrive in the village as part of a social activity course to improve villagers' health and hygiene. They are assigned to stay in poor families' houses and a campus-like building is reserved for Vishnu and the girls. The visit is designed to uplift the community, not to indulge in romance.

Vishnu and his friends begin trying to woo Divya and the other girls, misreading them as Thottapuram's sex workers. Divya and her peers decide to teach the boys a lesson instead of reacting with anger. The women trap the men in a plan to expose the deception.

The women lead the men to separate rooms where they inject a serum causing intense itching. The men scream and scramble, revealing their folly and underscoring Divya's resolve to reform them. Vishnu's image as a responsible suitor begins to crack.

Embarrassed and moved, Vishnu tells Divya he will save her honor by marrying her in the village temple the next day. Divya does not come to accept his proposal, leaving Vishnu disappointed. The moment reveals the deeper feelings and the fragility of the deception.

Divya is seen leaving on a bus bearing her college name, proving the romance was part of a trap. Vishnu witnesses the departure and begins to doubt the village’s intentions as well as the mission's success. The incident deepens his inner conflict about Divya and the plan.

Back home, Shivashankar learns that Vishnu has fallen in love. Under his influence, Vishnu and Divya become engaged, a union that seems to resolve the tension and unite the family. The engagement marks a turning point before the darker events unfold.

After a nightclub party, Vishnu goes to Divya's house in a drunken state. Divya's family prevents him from speaking with her, and a confrontational dispute erupts between the two sides. The incident foreshadows the violence to come.

Jeeva, Vishnu's younger twin, takes over Vishnu's identity and goes to Divya's cousin's home, where he attempts to rape her cousin. The assault reveals the extremity of the impersonation and deepens the family's peril. Shivashankar's fears for the wedding escalate.

Vishnu makes a bid to kill Shivashankar during the chaos, but Ko Thandam intervenes and is stabbed in the process. The near-fatal confrontation pushes Vishnu toward psychiatric intervention. Shivashankar realizes the severity of his son's actions.

Shivashankar has Vishnu admitted to a psychiatric facility to curb his violence. Meanwhile, Jeeva's impersonation intensifies the confusion and threat around the family. Divya, believing Vishnu to be innocent, becomes involved in the unfolding deception.

Jeeva, having stolen Vishnu's identity, escapes the hospital with Vishnu’s body and heads to kill Shivashankar. Divya sneaks into Vishnu's hospital room and, for a moment, believes he is innocent. The line between truth and impersonation becomes dangerously blurred.

The narrative cuts to a flashback detailing Shivashankar's past: a Bharatanatyam dancer who faced ridicule, his mother's death, and the rape of Gayathri that produced Vishnu and Jeeva. The revelation reframes the feud and the twins' motives. The audience learns why Jeeva seeks vengeance.

At the wedding-day showdown, Jeeva reveals himself as Shivashankar's son and aims to kill him. A police officer shoots Jeeva, misinterpreting the threat, and Shivashankar dies while trying to intervene. Jeeva is jailed as the family mourns the loss.

Weeks after the tragedy, Gayathri refuses food until Vishnu, disguised as Jeeva, feeds her. The final moment seals a fragile reconciliation and confirms Vishnu's loyalty to the family. The film ends with Vishnu declaring Shivashankar as the godfather of the family.
