Frank Galvin is a once-promising attorney who has been worn down by alcoholism and a career spent chasing ambulance to recoveries that rarely come easy. When his former partner, Mickey Morrissey, taps him to take on a medical malpractice case, Galvin sees a chance to prove himself again. The case centers on Deborah Ann Kaye, who fell into a coma after receiving general anesthesia during childbirth at a Catholic hospital. Her sister and brother-in-law want the suit to cover her ongoing care, making the stakes personal and urgent.

A Catholic diocese representative offers a settlement of $210,000, a figure that would make life easier for the family but not for Galvin. The moment he meets Kaye and witnesses the impact of her condition on those who love her, he refuses the offer and declares his intention to take the case to trial. His stance startles the defense team and the judge, setting up a high-stakes courtroom confrontation. Along the way, Galvin also meets and connects with Laura Fischer, a divorced woman he encounters in a bar, and their budding romance adds a layer of emotional complexity to his professional gamble.

The road to trial is rocky. A key medical expert vanishes, and a hastily arranged substitute’s credentials face scrutiny. The defense team, with substantial resources and a practiced press operation, makes it hard for any witness connected to the delivery room to testify that negligence occurred. The hospital’s attorney, Ed Concannon, commands a formidable legal apparatus that keeps pressure on the family and their supporters. Tensions flare when Kaye’s brother-in-law confronts Galvin after hearing of the rejected settlement, highlighting the growing frustration and risk of the case.

In a tense in-chambers exchange, Judge Hoyle challenges Galvin and threatens disbarment, while Galvin fires back, calling the judge a “Bag Man” for the local political machine and labeling him “a defendant’s judge” who could not hack it as a lawyer. Hoyle refuses to grant a mistrial and hints at legal consequences, prompting Galvin to storm out and reassess his strategy. The plot deepens as Galvin discovers that Kaye’s admitting nurse, Kaitlin Costello, completed a form asking when she last ate, and had raised questions about the timing of the meal.

Galvin travels to New York to secure testimony from Costello, while Laura arranges to meet him there. Morrissey discovers a presence of Concannon’s influence in Laura’s actions—his discovery of a check in her handbag confirms she’s acting as a spy for the defense. Morrissey travels to New York as well and warns Galvin of the betrayal. A tense confrontation follows between Galvin and Laura, which ends with a punch in a bar, underscoring the personal costs of the case. On the flight back to Boston, Morrissey pushes for a mistrial due to the ethics violation, but Galvin decides to press on with the trial.

In court, Costello testifies that she wrote on the admitting form that Kaye had eaten a full meal an hour before arriving at the hospital. Costello also testifies that the anesthesiologist later confessed he had missed the admitting notes and gave general anesthesia to a patient who had eaten too recently; the witness describes the pressure she faced to alter the record, including threats to her career.

Following Costello’s testimony, Concannon again argues about the weight of the original document, but Hoyle ultimately sustains the objection and has Costello’s testimony stricken from the record. A diocese lawyer then commends Concannon’s performance to the bishop, who asks, “Did you believe her?” prompting an awkward silence and a sense of unresolved doubt. Despite the feeling that the case might slip away, Galvin delivers a vehement and persuasive closing argument that resonates with the jurors.

The jury returns a verdict in favor of the plaintiffs, and the foreman asks whether they may award more than the amount sought. Hoyle concedes that they may. As the courtroom erupts in a mixture of relief and vindication, Galvin is congratulated outside the atrium, and he glimpses Laura watching him from across the space.
