Serafina Delle Rose, Anna Magnani, is a proud Sicilian seamstress living in a tight-knit community near the Gulf of Mexico. She cherishes her husband Rosario Delle Rose, Larry Chance, and is hopeful for the arrival of their second child, even as secrets and social pressures press in around them.
One night, Estelle Hohengarten, Virginia Grey, asks Serafina to sew a shirt for her lover from luxurious silk, unaware that the lover is Rosario and that Estelle wears a rose tattoo on her chest to match his. The request unsettles Serafina, who clings to a fragile trust in her marriage.
Tragedy intervene when Rosario is killed in an accident while trying to outrun police during a smuggling run. Serafina collapses, and Rosa Delle Rose, Marisa Pavan, is told by the doctor that she has miscarried. The shock fractures Serafina’s world, and the family’s grip on its quiet dignity is forever altered.
Three years pass. Serafina has withdrawn into a guarded, reclusive life, letting her appearance and reputation slide. She attends Rosa’s graduation, but a pair of prostitutes arrive demanding she mend their bandanas for a political convention. The encounter unsettles her, and one of the women, Bessie, Jo Van Fleet, openly mocks Rosario’s alleged infidelity. Rosa introduces her new sailor boyfriend Jack Hunter, and he, Ben Cooper, asks Serafina to bless their impending marriage before a statue of the Virgin Mary.
Determined to uncover the truth about Rosario, Serafina seeks out a priest to ask if Rosario ever confessed to an affair. When the priest declines to answer, a truck driver named Alvaro Mangiacavallo, Burt Lancaster, intervenes and pulls her away. He drives her home, and she offers to repair his torn shirt, even loaning him the rose silk shirt she had sewn on the night of Rosario’s death until she can fix his. The two are drawn together, and Alvaro tattoos a rose on his chest after a moment of impulsive self-definition. They agree to meet again later that night.
Alvaro returns, and Serafina is initially repelled by the gesture, tossing him out before she confronts the deeper ache behind his bravado. She takes him to a club that once belonged to Rosario, where Estelle reveals and confirms her own rose tattoo as a symbol of love for Rosario. Back home, Serafina smashes Rosario’s ashes in a sudden, furious act and invites Alvaro to return.
That night, Alvaro comes back intoxicated and is left to sleep it off as Rosa returns home and falls onto the couch. Believing she’s Serafina, Alvaro tries to kiss Rosa, who screams, and Serafina swiftly removes him from the house. The morning after, Alvaro is seen on a boat mast outside Serafina’s home, pleading for forgiveness. Jack arrives and asks Serafina to give Rosa away in marriage, and she grants her blessing.
In a defiant, hopeful turn, Serafina signals that life must go on as she calls Alvaro down from the mast and, in front of their neighbors, declares that they must pick up where they left off. The house fills with music from a player piano—a lively pop tune—and the sounds of celebration and laughter as the new arrangement of their lives begins.
