Set in the exacting shadows of the Victorian era, Guy Carrell, a British aristocrat, is relentlessly haunted by the fear of being buried alive. He warns his betrothed, Emily Gault, that he—like his father—may suffer from a cataleptic condition that could mimic death. To prove his point, he leads her to the family catacomb and recounts how, as a boy, he heard his father scream from within the tomb, even though his sister, Kate Carrell, insists it was all in his mind. Despite the bone-deep chill of this history, Emily remains determined to wed him.
After the ceremony, Emily sits at the piano and plays the mournful melody of “Molly Malone,” a tune that seems to plunge Guy into a state of abject misery and sends him into a faint. When he regains consciousness, his fixation on death hardens into morbid resolve. He undertakes a meticulous project: a burial vault built with multiple safeguards against premature burial, complete with a poisonous elixir as a last resort. His escalating paranoia alarms his close observer, Miles Archer, and his physician, Dr. Gideon Gault, who worry about his mental health.
In an effort to lift his mood, the couple strolls over the moors, but the moment is punctured by the whistle of a gravedigger, the same Irish tune that tormented him earlier. The sound triggers a terrifying dream in which he finds himself trapped inside his vault, with every safeguard failing. He awakens beside Emily, who swears she heard no one. The episode deepens the couple’s rift until Emily issues a stark ultimatum: abandon this death-obsession or she will leave him forever. The threat seems to break the spell, and Guy destroys the vault, attempting a kinder future.
Miles then proposes a drastic, clinical test: open his father’s tomb to prove he was never buried alive. The audacious plan drives Guy into another cataleptic state, and when he awakens later, doctors declare him dead. The family buries him in the cemetery, and for a moment it appears his deepest fear might be actualized. Yet fate has more torment in store: he is miraculously dug up by a pair of grave robbers just as his body regains mobility, throwing him into a fury of renewed madness.
Back at home, Guy targets Emily’s father in the laboratory, electrocution delivering a fatal blow. Amid the chaos, Emily tells Miles she has real feelings for him and confesses she made a mistake marrying Guy. A servant summons Miles, and Guy confronts Emily in her room, where she faints. He drags her to the graveyard and places her in his burial plot, covering her with earth. Miles pleads for mercy, and a struggle ensues. Kate Carrell arrives and ends the violence by shooting Guy dead on the spot.
Miles retrieves Emily, only to find that she has died. In a shocking confession, Kate reveals that Emily orchestrated Guy’s demise: she hired grave robbers to torment him, hid a cat within the walls of the house, and even desecrated his father’s tomb to fuel his fear—yet waited to reveal her treachery until too late.
