An unnamed male corpse, referred to as John Doe, is brought into a hospital morgue after dying in an ambulance from an apparent suicide. This event sets off a mysterious chain of occurrences as the story begins to unfold.

In a shocking turn of events, John Doe comes back to life and staggers out of the morgue. He then proceeds to an adjacent hospital room and falls asleep in a bed, leading to more questions about his condition.

Dr. Edward Graham, a medical investigator, is summoned to the morgue where he discovers that John Doe's body has mysteriously disappeared. This prompts an urgent investigation into the odd circumstances surrounding the case.

The next morning, nurses find John Doe in a catatonic state, his body marred with scars. This raises alarms among the medical staff about his mental and physical condition.

Dr. Daniel Forrester, the hospital psychologist, is called to evaluate John Doe. He manipulates the situation with false claims of self-harm, aiming to admit John Doe under a 5150 hold for further observation.

As Travis checks on John Doe, John suddenly attacks him, leading to an emergency situation. Forrester intervenes, sedating John with benzodiazepine, but strange hallucinations begin to plague both Travis and John Doe.

Graham visits the motel where John Doe died and uncovers a spiral pattern marked into the bathtub. He also finds John Doe's driver's license, hinting at deeper mysteries related to his past.

Forrester attempts to hypnotize John Doe, provoking a return of the disturbing ringing in his ears. John reveals that he brought something back from a near-death experience that is now tormenting him.

Graham discovers John's identity as Michael Clark and reads his chilling suicide note. It expresses a sense of despair and a desire for forgiveness, hinting at the horror that lies ahead.

Escaping his restraints, John Doe attacks another patient, sucking a mysterious black smoke from her mouth. She is later found dead, her expression frozen in horror, connecting various victims to his dark powers.

At Clark's family home, Graham finds distressing evidence related to a house fire that killed John’s wife. This discovery leads him to a disturbing collage of mass death events that ties to the current horror.

During a sodium thiopental interrogation, John pleads for Forrester to end his life, revealing that the creature inside of him is growing stronger. As he becomes increasingly unstable, Forrester struggles to manage the situation.

After being criticized by Dr. Grey, Forrester attempts to attack John, only to be subdued. He later discovers that Dr. Grey has become another victim in this terrifying sequence of events.

Forrester arrives at Clark's family home only to find tragedy as Clark has killed his parents and children along with Graham. The horrifying scene intensifies the urgency to stop Clark.

In a climactic showdown, Forrester battles Clark, who overwhelms him with the dark powers within. However, Forrester ultimately regains control and defeats Clark, but is left with dark marks on his face as a sign of the evil remaining.
