In 1944, in the wake of Puppet Master III: Toulon’s Revenge, Toulon and his little troupe hide away in the Kolewige, an inn not far from the Swiss border. A chance discovery jolts the quiet: Blade finds the wooden head of an ancient puppet named Cyclops tucked among their belongings. To explain what he has lived through, Toulon retells a sprawling history that stretches back to Cairo, Egypt, in 1902, where the seeds of his power first took root and where his memories of a woman he loved began to intertwine with the mysterious, living puppets.
Long before the inn’s hearth grows cold, a 3,000-year-old Egyptian sorcerer named Afzel steals the secret of life itself, fleeing from the reach of a dark god named Sutekh. Afzel’s pursuers, two magical servants, press the chase but are easily defeated by Afzel’s own formidable power. Afzel makes his way toward Paris, setting in motion a chain of events that will cast a shadow over Toulon’s future. In parallel, Sutekh breathes life into three ancient Egyptian mummies, awakening them to hunt Afzel and reclaim what has been stolen.
In Paris, a young André Toulon takes to the stage, presenting a puppet show based on Dante’s Divine Comedy. Among the spectators is Elsa Elsa, who has fled a harsh, controlling father to witness art’s enchantment firsthand. In the city’s shadowed sewers, the three mummies enlist two hired thugs to kill Afzel, aware that Afzel can sense their approach and presents a dangerous obstacle to their plan. Afzel is violently beaten, but Elsa’s arrival interrupts the assault, helping to pull Afzel back from the brink of death and into a safer space. Afzel, waking, reveals that he knows the secret of life—the very knowledge that could shield humanity when the elder gods rise again in eons to come—and offers to pass it on to Toulon.
With Afzel’s blessing, a strange ritual unfolds: the beggar who has sat on the steps since the start of the tale is moved into the puppet Pinhead via a magical ring, his soul transplant animating the new performer. The beggar’s spirit gives Pinhead motion, though the puppet’s quick questions cause it to flee back into the theater. Elsa returns to speak with Toulon, but her father’s stern, protective guards interrupt the moment, dragging Toulon away and leaving Elsa in peril. The next day, in the theater, the barrier Afzel etched on a sheet of paper begins to fail, and the mummies burst into a full-blown assault. In the chaos, Valentin rushes to repair the door while the mummies strike, killing one of the accompanying humans and wounding others. Vigo falls backstage, and Duval fights valiantly but is slain as the mummies strike from multiple directions. Latour draws a gun and shoots the lead mummy three times, but the other two still overwhelm him. Afzel reappears and proclaims that Sutekh will not claim more lives, then sacrifices himself with a surge of magic, vanishing as the battle ends.
With Afzel’s death, Toulon acts quickly, seizing the opportunity to transfer the scattered souls into his puppets, intent on protecting Elsa and the world from the looming threat. Six-Shooter, one of the henchmen, triggers a fatal trap by loosening chandelier chains, crushing the lead mummy and forcing the remaining henchmen to retreat and regroup. Elsa is then seized, and the two men pursuing Toulon—now threatened by both human enemies and the magical dead—seek to lure him into a trap through Elsa.
Realizing the danger around them, [Andre] and his puppets set out to reach safety at the Calais train station, only to discover that danger has followed. Elsa is captured, and a prophetic dream shows her bound and waiting aboard a train. Toulon makes a crucial choice, swapping trains for Marseille and preparing his living puppets for a final confrontation. As the last car of the train comes into view, Elsa lies bound and the two henchmen converge, eager to learn the secret of life from the legendary puppeteer. The ensuing struggle is fierce: Toulon’s puppets spring into action, and the secrets held within the scroll—how to unlock life itself—are put on the line. In the climactic moment, the remaining lead pursuer is hurled from the carriage as a brutal fight ensues, enabling them to free Elsa and ride away into the uncertain horizon.
