Cassie Keller Emily Osment is a goth girl who has just moved with her family to a new neighborhood and school. She loves to play scary pranks on the popular kids at school and on her younger brother, Max Keller Alex Winzenread. The mood in her new environment feels tense and sometimes lonely, but Cassie isn’t afraid to push boundaries to protect herself and push back against those who lash out at her.

At the school Halloween fair, Priscilla Wright Brittany Curran, a notorious bully, is elected Halloween Queen. Cassie’s sharp wit meets Priscilla’s rude remarks, and the tension spikes into a reckless plan: Cassie secretly fills Priscilla’s piñata with live cockroaches, so when she breaks it, she is showered in insects and publicly mocked by the student body. The prank reveals Cassie’s willingness to confront cruelty, even as it pushes her further into a world where ordinary high school drama begins to blend with something darker.

In a narrow, shadowy alley, Cassie discovers a mysterious Halloween store. The owner, Tobin Bell as the Stranger, insists on selling her an old book titled The Evil Thing, which carries a chilling warning not to read it aloud or even think about “the Evil Thing,” a two-headed creature capable of both bloodsucking and flesh-eating. The warning leaves a tremor of doubt, but Cassie’s desire to get even with the world’s cruelties drowns out caution, and she opens the book that very night.

That same night, Cassie reads the book aloud to Max, a punishment for him unplugging her computer. The ritual awakens the Evil Thing, which is brought to life by Max’s thoughts and swiftly closes in on its victims, capturing Max, Priscilla, and a Papa John’s deliveryman. The danger becomes immediate and personal, forcing Cassie to seek help beyond the confines of the classroom and the neighborhood streets.

Desperate for guidance, Cassie returns to the mysterious store and finds that the Stranger travels the world each Halloween in search of someone who loves to scare others—the kind of person who can unwittingly unleash a powerful entity by reading the book. He leaves them with a cryptic, dangerous riddle: > Two heads are better than one; that’s the way to get the bloody job done.

Armed with this riddle, Cassie and Sean Cody Linley—the popular boy she secretly likes—form a fragile alliance to outsmart the monster. Their plan hinges on taking advantage of the Evil Thing’s peculiar biology and the danger it poses to the people they care about. They realize they need a very specific kind of catalyst to turn the creature against itself.

A crucial piece of the plan involves blood from a roast that Cassie’s mother, Eileen Keller Michelle Duffy, made. The idea is to anoint the Evil Thing with this blood in a way that will trigger the two heads to attack one another, potentially forcing the monster to devour itself. Alongside the strategic move, the Evil Thing’s babies hatch, complicating the rescue attempt as the group scrambles to keep Priscilla and the pizza deliveryman safe.

Max, bolstered by Cassie’s encouragement, must confront his fears to play his part in the plan. The moment of action comes when Max, inspired by Cassie’s resolve, hurls the blood onto the creature, and the two heads turn on each other in a deadly internal struggle. The result is catastrophic for the monster: it devours itself, exploding in a shower of yellow blood and ending the immediate threat to the survivors.

With the threat temporarily contained, Cassie, Sean, and Max rescue Priscilla and the pizza deliveryman. They burn the book, trying to sever the link to the Evil Thing and set aside thoughts of vengeance. Yet their relief is short-lived, because the family returns home to an unsettling discovery: the burned pages have a last spark of life. The father reopens the scene by reading the book aloud, reviving the Evil Thing once again. In the end, Cassie realizes — with a wary sense of foreboding — that the Evil Thing has been brought back into the world, leaving the door open for a renewed confrontation in the future.
