In 1958, Ferren Woods, a quiet backwater town, bears witness to a grim origin when an old blind witch, Ms. Osie, Lilyan Chauvin feeds a deformed orphan named Tommy J.P. Manoux; he is the offspring of Pumpkinhead Mark McCracken. As Tommy eats, a car of six thugs rolls into view, misreading him as a demonic monster and pursue him with switchblades and bats. They corner him at an ancient iron mine, forcing a brutal, lethal assault that leaves him bludgeoned, slashed, and dropped into the dark shaft below, where he dies in cold, deliberate fashion.
Thirty-five years later, in 1993, Sheriff Sean Braddock Andrew Robinson returns to Ferren Woods with his wife and daughter Jenny Ami Dolenz. Sean grew up here, and his old friendship with Tommy shadows his present life. At school, Jenny crosses paths with a rowdy group of teens, including Danny Dixon J. Trevor Edmond, whose father once stood as a town judge. One night, the teens pilfer Sean’s car, and Danny accidentally strikes Ms. Osie. They hurry to her cabin to check on her, discovering a spellbook and vials of blood, tools Ms. Osie uses to resurrect legends. When Ms. Osie confronts them, Danny knocks her down and escapes with a vial, leaving the elder woman to a perilous fate.
Desperate to learn what happened to Ms. Osie, Danny and his friends attempt to resurrect the dead Tommy, hoping to conjure something beyond the ordinary. Jenny notices Ms. Osie’s cabin catching fire, and Danny and his companions flee as the old woman burns, ending up in the hospital though her spell has unintended consequences. The ritual proves successful in reviving Tommy not as a boy but as the monstrous Pumpkinhead Mark McCracken. Before long, Judge Dixon’s circle of friends begins to fall to grisly, supernatural violence.
Jenny’s father investigates the strange string of murders, and Ms. Osie’s death leaves behind cryptic clues. Sean starts to connect the dots, realizing the killer is linked to Pumpkinhead and that the judge himself may be next. When Judge Dixon gathers his posse to hunt whatever is behind the carnage, Pumpkinhead strikes down the judge, ending his side of the town’s old feud and avenging the misdeed that haunted him for decades.
With the demon’s appetite for vengeance satisfied, Pumpkinhead—Tommy Tommy by his monstrous form—turns toward Danny and his friends, who have already fallen to his wrath. Danny Dixon J. Trevor Edmond and his companions are slain in brutal fashion, leaving the woods punctuated by fear and silence. Jenny is chased toward the iron mine, while Sean and a town doctor press on to find her, guided by the faint memory of a debt he once owed to the boy who became this revenant. Because Sean had saved Tommy’s life years earlier when he was a child, Pumpkinhead grants Jenny safe passage to her father and spares her for the moment.
