An unknown outbreak devastates the United States as airstrikes begin. Ray, Claire, and Zoe retreat to an abandoned fallout shelter, turning it into their makeshift home. They struggle to adjust to cramped conditions while trying to keep hope alive.

To cope with scarcity and danger, they set a routine and rules designed to stay calm and stay quiet. They ration food, monitor noise, and reinforce their barricades in the dim shelter. The family tries to cling to normalcy amid the growing threat outside.

Claire kills a rat that has been consuming their dwindling rations. In the process, a lamp is knocked over and a small fire breaks out, emitting smoke that drifts into the air duct. The smoke makes their hiding place more conspicuous and hazardous.

The accumulating smoke coats the hatch with ash, giving away their position. Zoe, using a homemade periscope, glimpses a humanoid figure in the distance. Ignoring the rules, she climbs to the surface to warn her parents, but the Breathers close in and a doll’s noise betrays their presence.

In a flashback, the family flees along a highway out of Kingsville, only to be stopped by the CDC announcing quarantine. Signs warn the population and the escape becomes a struggle against government containment. The fear of what happened outside haunts them throughout the shelter's walls.

More Breathers gather outside and manage to breach the shelter hatch. Claire and Zoe escape through the air duct, while Ray is captured and disappears, leaving behind a trail of blood. The threat outside grows as they reach the highway area.

In another flashback, two military jets fly overhead and bomb the family's town. They rush to an abandoned shelter as the rest of the town is decimated by the explosions. Claire cuts herself on rusty iron, and they realize the virus is airborne and they have already been infected.

Back in the present, soldiers test Claire and Zoe's blood and interrogate them to determine if there are others nearby. The situation grows dire as the soldiers prepare to execute them. Ray suddenly appears and kills one soldier, revealing his unusual strength.

Ray incapacitates several soldiers with brutal force but is shot multiple times and collapses. Claire and Zoe manage to escape from their net, while Ray dies in the chaos. The family loses a crucial protector as the threat closes in.

In a feral, reflexive defense, Claire transforms and kills numerous soldiers with almost inhuman ferocity. She is eventually wounded by a remaining soldier, showing how the infection has reshaped her body and actions. The moment pushes the trio toward a desperate escape.

Zoe, fueled by anger, removes a soldier’s re-breather and exposes him to air, aiming to infect him. She nearly attacks her mother in the chaos, but Claire calmly reminds her of their rule to stay calm. The tension between mother and daughter peaks here.

Claire and Zoe escape the ruined shelter and slide into a nearby sewer system. They are guided by Joey, Zoe’s friend, who leads them to others who have survived in the underground. Zoe notes that they’ve reached day 302, highlighting the miraculous nature of their survival.
