A pregnant woman named Anna flees a pursuing cluster of infected, but as she gives birth to Ellie, she is bitten, infecting Anna before she can cut Ellie’s umbilical cord. The newborn remains unharmed, yet Anna is doomed by the bite. A rescue party led by Marlene, alongside a group of Fireflies, finds them in the hushed aftermath. Anna begs Marlene to kill her and take Ellie back to Boston, twisting the truth to say she was bitten after the cord was cut. After a tense pause, Marlene takes Ellie and ends Anna’s life, a decision that shadows the child’s future with a heavy, unspoken debt.

In the present, Joel and Ellie arrive in Salt Lake City, Utah, seeking the Fireflies’ hidden laboratory. Ellie, still haunted by her encounter with David, moves with a restrained stillness that only loosens when a wandering herd of giraffes crosses their path, a surreal moment that briefly lifts her spirits. Joel, sensing the burden they’ve carried, suggests turning back to Tommy’s settlement in Jackson, Wyoming, hoping to abandon the dangerous quest. Ellie, however, speaks with a tempered resolve, insisting that they press on to finish what they started. In a moment of candor, Joel shares the weight of his past, including a suicide attempt after Sarah’s death, and invites Ellie to ease the journey with a few jokes from her book.

Their uneasy progress is halted by an ambush from Firefly soldiers, and Ellie is captured while Joel is knocked unconscious. When Joel regains consciousness, he learns from Marlene that doctors believe Cordyceps has grown in Ellie’s brain since birth, granting her a unique immunity. He quickly understands the Fireflies’ plan: remove Ellie’s brain to forge a cure, a mission that would kill her. Marlene tries to restrain him, but Joel moves with swift, ruthless resolve, overcoming guards and cutting a path toward the operating room. He confronts the lead surgeon who resists giving Ellie up and, in a decisive moment, shoots him. He then abducts the unconscious Ellie and makes for an escape, only to be confronted by Marlene in the parking garage. She urges him to reconsider, hinting at a future where Ellie might still be pursued if spared; Joel shoots Marlene, ending her warnings and any chance she offers of mercy.

Ellie awakens as Joel drives away from the city, and he fabricates a grim lie: the Fireflies had found other immune individuals but failed to produce a cure, forcing them to escape after the hospital was attacked by raiders. With their car soon breaking down, the pair shoulder the long trek to Jackson on foot, the road ahead uncertain. Ellie, grappling with survivor guilt, presses Joel to tell the truth, and he swears that his story is valid; she pauses, tests him with a quiet “Okay,” and the weight of their shared burden settles between them.
