A satanic cult in a Guanajuato mine constructs a hidden temple and performs rites that require the removal of victims' left hands. These severed hands become a cursed relic tied to the Devil. The cult's grisly rites plant the seed for the horror that will surface centuries later.

In the present day, Mark and Jennifer Baines explore the temple and discover a small casket containing a severed left hand. They haul the ominous relic back to their hotel room, unaware of the danger it carries. The hand's dark influence begins to take hold as soon as it's disturbed.

That night, the moment Mark opens the casket, the severed hand clamps onto him and takes control. He experiences a surge of strength and a frightening loss of free will under its grip. Jennifer senses something is wrong as Mark's eyes change and his voice grows cold.

Fleeing the temple, Mark travels to Las Vegas while still possessed. He suddenly wins a large fortune at the gambling tables, seemingly guided by the hand. The win only deepens the possession and the danger he represents.

Desperate to end the possession, Mark tries to sever his left hand himself. The hand resists and, during the struggle, burns him to death with an otherworldly blaze. The burned body is left behind as the hand continues to act elsewhere.

Mark's severely burned corpse is shipped to Los Angeles for burial, leaving Jennifer to confront the nightmare alone. The authorities suspect nothing unusual at first, but the corpse's odd burns hint at something supernatural.

Jennifer goes to Father Cunningham's church and warns him that Mark's body may still be possessed. She requests an autopsy to determine if the haunting has not ended with death. Cunningham agrees to consider the request and prepares to act.

During the burial preparations, Mark's charred corpse reanimates and bursts from his coffin, escaping into the night. The terrifying revival confirms that the hand's influence survives beyond death.

Police Sergeant Leo Matson arrives to investigate and is suddenly attacked by Mark. The possessed left hand slams through a door, severing Matson's hand, and Mark collapses dead as the hand immediately possesses him. Matson's transformation into a host begins a new chapter of danger.

The hand possesses Matson and drives him to a plastic surgery office. He forces Dr. Julian Rivkin at gunpoint to sever the hand, and the hand then shoots a nurse and murders Matson by ripping his face off. The possession shifts to Rivkin as his own hand becomes the next vessel.

The hand possesses Dr. Rivkin, who severs his own hand on a train track. The spree of carnage continues as the possessed doctor becomes the hand's next agent.

The hand corners Jennifer at her motel and again attempts to possess her, but Father Cunningham intervenes and helps them flee toward the church. They race between locations as the hand closes in.

Inside the church, the hand briefly possesses Cunningham and attacks Jennifer with a knife. Cunningham manages to break free by stabbing his own hand, then Jennifer and he burn the hand off with a blowtorch, scattering its ashes in the harbor.

Jennifer returns home, but the surviving hand finds her and attacks again. The hand kills her, ending the nightmare.
