The film begins with Richland (Feore) sent to a secluded government facility on a remote island. He follows Peniston (Esposito) into the facility, where head scientist Dr. Bishop (Reineke) showcases several groundbreaking projects, including a shark-dolphin hybrid capable of adapting to diverse environments, and a chilling human-shark hybrid referred to as the Creature. As events unfold, the creature escapes from its enclosure and fatally wounds Dr. Bishop. In a desperate bid, Richland commands Peniston to eliminate it, but Peniston instead confines the creature in a containment unit and discards it into the ocean.

Fast forward 25 years… Dr. Chase (Nelson), a dedicated marine biologist, along with his assistant Tall Man (Williams), discovers a pregnant female Great White Shark entangled in the nets of Ben Mediera (Alyward). They successfully liberate it, but unbeknownst to them, local fisherman Puckett (Burke) retrieves the discarded containment unit, inadvertently releasing the monstrous creature. Tragedy strikes when Mediera is claimed by the creature.

At the airport, Dr. Chase reunites with Dr. Macy (Cattrall), her sea lion companion Robin, and his son Max (Carey). As they prepare for an ocean expedition, Max ventures into town and encounters local youths, including the chief’s daughter, Elizabeth (Echikunwoke). The local boys challenge him to demonstrate bravery by leaping off a high cliff.

As the investigation into Mediera’s death progresses, police Chief Gibson requests Dr. Chase’s expertise concerning the body. Despite the presence of the Great White, Dr. Chase insists it is not the perpetrator and pleads with the chief to refrain from pursuing the innocent shark. Yet tension escalates as Dr. Chase and Dr. Macy mobilize to uncover the true threat. Meanwhile, Peniston, now labeled ‘Werewolf’ by locals due to his erratic state, realizes that Mediera was not slain by the shark after he sees the empty containment unit.

Dr. Chase and Dr. Macy’s search leads them back to the old research facility, where Dr. Macy reveals that they are studying why sharks are immune to cancer — a quest born from Chase’s best friend’s tragic death. As they delve deeper, local kids recklessly jump from the cliff, unaware that the creature lurks beneath the surface.

When the body of Kimo, a boy who met a gruesome fate, is discovered, the chief reacts with fury, dismissing Dr. Chase’s warnings and inciting a furious hunt for the Great White. Allegations against Chase and Macy reach a fever pitch, accusing them of witchcraft. Determined to clear their names, the pair, along with Max and Tall Man, set out to confront the creature once again, only to find themselves in peril when it viciously attacks their boat.

Despite Puckett’s success in killing the Great White, the jubilation is short-lived as Dr. Chase warns of greater danger; he has seen the real creature. Upon learning of Richland’s arrival, who advises against action, Dr. Chase defies orders, risking everything to protect the island. With the creature proving it can be lured by noise, brutal encounters ensue, leading to devastation back at the research facility as the creature grows stronger and more cunning.

In a race against time, the heroes navigate through treacherous tunnels filled with debris, eventually uncovering the horror of what Richland had concealed. Following intense exchanges and near-fatal encounters, the group devises a bold plan to contain the creature, luring it using sound. Helplessly, they witness their friends fall victim to the monster’s wrath.

Amid a chaotic climax, it is only through resourcefulness and sacrifice that they manage to culminate the creature’s reign of terror. When the airlock explodes, dismantling the creature once and for all, the survivors emerge from the carnage into the light, their fateful adventure leaving them changed forever. As the sun sets on the island, they prepare to return, knowing they will never look back.
