Clay Riddell arrives at Boston airport excitedly contacting his wife, Sharon. He shares his joy over securing a deal for his graphic novel, but their conversation quickly reveals underlying tensions, echoing their fractured relationship.

While Clay attempts to reach out to Sharon again, chaos ensues triggered by the mysterious dissonant sound affecting cell phone users. This sudden crisis leads to violent outbreaks across the city, culminating in an airplane crash.

Clay barely escapes the raging chaos through a subway entrance. He encounters a conductor named Tom and several survivors, and together they decide to venture down dark subway tracks amidst the danger lurking around them.

As Clay and Tom trek through the tracks, they encounter horrific, zombie-like figures known as 'phoners.' After a dangerous confrontation, they manage to escape through a manhole, discovering Boston in ruins.

Reaching Clay's apartment, they witness a terrifying event outside involving a motorcycle rider attacked by phoners. Clay reflects on his unsettling artwork that eerily depicts the movie's antagonist, deepening his sense of urgency.

Their isolation is breached by their neighbor, Alice, who appears bloodied and desperate for refuge after her mother's murder. The trio forms an uneasy alliance as they attempt to navigate the violent new world together.

The trio explores the surrounding area, discovering a grim scene near a cell tower with deceased individuals holding firearms. Their initial looting leads to a shocking transformation as an individual metamorphoses into a phoner before their eyes.

Under the cover of night, they reach Gaitan Academy and meet its Headmaster, Charles, and a student, Jordan. Their sense of safety is shattered upon discovering a large gathering of phoners resting on the soccer field.

As Charles theorizes about the evolutionary implications of the phoners, the group debates their next steps. In their attempt to fight back, a chaotic explosion occurs, tragically claiming Charles's life and testing the group's resolve.

As snowfall blankets Boston, the group seeks shelter at a drive-in theater. Here, Tom delivers a reflective sermon on sin, while Clay grapples with unsettling dreams that draw parallels to his haunting artwork.

A feeling of safety shatters when Sally, one of the group members, turns into a phoner after receiving a signal. In the ensuing chaos, Alice suffers a fatal injury, leaving Clay and the remaining members heartbroken and resolute.

The group survives a harsh reality and meets Ray and Denise, who offer advice about the phoners. Ray alludes to a deeper mystery involving a mysterious 'man in red,' hinting at greater dangers that lie ahead.
