Diana Joseph successfully apprehends members of the Sikka Gang and confiscates a massive haul of cocaine valued at ₹1,000 crore. This marks a significant victory in her crackdown on drug trafficking operations. The operation sets the stage for the escalating conflict and reveals the gang's involvement.

Abbas Ali, known as Raunak, joins Ashwathama 'Ashu', the co-leader of the Sikka Gang, as an undercover officer. He provides vital intelligence about Ashu’s activities to Diana. This hidden alliance influences subsequent events and the pursuit of justice.

Ashu seeks assistance from corrupt NCB officer Devraj Subramaniam, who orders Deep Singh to add rohypnol to drinks at IG Jayant Malik's farewell party. As a result, most officers fall unconscious, including the sedated officers being transported later.

Diana tries to persuade Bholaa, a released prisoner and former gangster, to drive a truck transporting the sedated cops to the hospital and police station. After initial reluctance, Bholaa agrees, setting their plan into motion amidst dangerous gang attacks.

While Bholaa, Diana, and Kadchi travel to the hospital, they are ambushed by rival gangs. Deep Singh provides Lima updates from hiding, and Bholaa fights off gang members, killing several assailants in the process.

Ashu dispatches his gang to assault the Laalganj police station and free his brother Nithari, who is caught with the confiscated cocaine. Constable Angad Yadav, newly transferred, is tasked with defending the station and confronting the attacking gang.

Ashu manages to breach the police station but is ultimately captured following resistance from Yadav and students. One student, Chetan, is killed when Ashu's gang strangulates him, and Ashu is shot dead by Yadav during the chaos.

Bholaa recounts his background as a notorious gangster in UP, his love for Dr. Swara, and her tragic death at the hands of gangster Chomu Singh. Swara's sacrifice and the birth of their daughter Jyoti leave Bholaa with deep emotional scars and a prison sentence.

Diana shares her heartbreak over losing her unborn child while she was pregnant, highlighting her emotional struggles. Her personal pain adds depth to her character as she continues her fight against crime.

Bholaa wakes up and single-handedly kills the remaining gang members, including Deep Singh, using a Trishul. He then rescues Diana from Bhoraa, who had arrived with more henchmen, and they escape using a secret passage after daytime battles.

Diana burns the confiscated cocaine as Ashu’s reinforced gang breaches the station. Bholaa destroys them with a M134 Minigun. Nithari dies shock from the chaos, and Devraj Subramaniam commits suicide upon discovering Bholaa's involvement.

Bholaa reunites with his daughter Jyoti, and they leave the station in a truck with Kadchi and a puppy, symbolizing a fresh start after the intense conflict. The story ends with a sense of hope and resolution.

In the mid-credits scene, the ruthless gangster Chomu Singh learns about Bholaa’s release from prison. He sets out to hunt Bholaa down, indicating future conflict and revenge.
