In the quaint town of Smithville, Texas, Vaughn Ely is more than just a friendly face; he harbors a sinister secret. Once a celebrated star quarterback, he now leads a grim existence as the town's mortician, a role that brings him closer to death than life.

Vaughn's life takes a treacherous turn when he discovers that his wife, Rosemary, is having an affair with David Moore, the partner of her former high school English teacher. Consumed by betrayal and rage, Vaughn's psyche begins to unravel as he contemplates revenge.

In a fit of rage, Vaughn commits an unimaginable act by murdering Rosemary. Rather than face the consequences, he keeps her lifeless body in his home, treating it as if she were still alive, which spirals him further into madness.

Two years later, four friends—Travis, Danny, Brian, and Abby—decide to sneak into Vaughn’s home, naively believing he is away. Their thrill turns to horror when they catch Vaughn in the act of a macabre dance with Rosemary's corpse.

Upon realizing the intrusion, Vaughn's rage is unleashed upon Danny, who suffers a brutal fate as Vaughn violently dispatches him down the stairs. Travis is left horrified, witnessing the gruesome act unfold before him.

Despite the traumatizing event, Vaughn chooses not to press charges against the friends. However, when they seek help from the police, their desperate claims are dismissed as mere pranks due to Vaughn's respectable façade.

Determined to reveal Vaughn's hidden darkness, Travis and Abby embark on an investigation into Danny’s death. Their pursuit leads them back to Vaughn’s home under cover of night, where they encounter the sinister reality of his living arrangements.

Things take a turn for the worse when Vaughn captures Abby, unconscious, and places her inside a casket buried in his backyard. Meanwhile, Travis suffers a gunshot wound during the chaos, heightening the urgency of their plight.

As Travis recuperates in the hospital with police supervision, his friend Brian devises a plan to help him escape. Misunderstandings and chaos ensue as the police mistakenly pursue Brian, allowing Travis a fleeting opportunity to flee back to Vaughn's home.

Travis returns to find Vaughn’s plans come to fruition as he is taken to a cemetery, forced to dig his own grave. In a moment of desperation, Travis urges Abby to flee, hoping she can avoid their grim fate despite Vaughn's relentless intentions.

Just when hope seems lost, Abby escapes Vaughn's clutches but is determined to rescue Travis. Clad in Rosemary's clothing, she confronts Vaughn, unleashing her pent-up fury and plunging into a chaotic struggle for survival.

Unfortunately, their victory proves short-lived as Vaughn is exhumed from his grave and confined to an insane asylum. His fate takes a dark turn as he grapples with his fractured psyche amidst the horrors he has endured.
