Babu Bihari operates as a hitman for both of his rivals, Dubey and Jiji, in Uttar Pradesh's volatile underworld. He charges ₹20,000 per hit and passes ₹5,000 to the constable who arranges the job. He lives a simple life in a remote shanty, keeping a low profile while maintaining his lethal reputation.

Jiji assigns Babu to kill a local muscleman. While surveying the target, Babu meets Phulwa, a determined cobbler, whose presence unsettles him. He is attracted to her but she rejects his advances, and he carries on with the job as she watches.

Phulwa witnesses the killing. Babu confronts Phulwa, and she stabs him in self-defense. He learns the victim and his brothers had raped Phulwa, and she implores him to kill the remaining brothers, which he does.

Upon Babu's return, Jiji erupts with anger because his actions have disrupted her political alliance with the brothers. The confrontation heights tensions between their empires and threatens his standing in the underworld.

Later, Babu and Phulwa have sex, signaling a personal bond forming between them. The act complicates loyalties and foreshadows the betrayals to come.

Dubey hires Babu to eliminate Jiji's three men. Babu informs Jiji and challenges her to protect them, turning the mission into a dangerous competition with Banke, a fellow admirer of Babu who joins in. They agree to a head-to-head contest: whoever kills more targets wins and the other quits.

Babu and Banke take turns attempting the kills; Babu claims one target while Banke takes the second. Babu is injured and cared for by Phulwa. To test loyalties, Babu offers Phulwa and Banke a night with him, but they refuse.

The pair finally kill the third target, Triloki, and celebrate their victory. Banke then reveals there was a fourth target, and he shoots Babu, sending him into a passing train.

Babu wakes up in a hospital with long hair and a beard, suggesting a long coma. He returns home and learns from a cop friend that after Triloki's death Jiji gathered her forces, burned his house, and Phulwa was killed. Babu is heartbroken and swears vengeance.

To avenge the losses, Babu locates Banke, who explains that he only carried out his contract and warns that Dubey had revealed Babu's address to Jiji. Babu spares Banke, but the revelations push him into a brutal killing spree against his enemies.

The secretary of Dubey offers Babu one more contract, which he refuses. He travels to a final address and discovers an astonishing reunion: Phulwa is alive with his son, and Banke has arrived as well. It is revealed that Phulwa and Banke fell in love and Phulwa had given Banke the contract to kill Babu.

To avenge the betrayals, Babu tricks Banke into a deadly game and kills him. He then kills Phulwa for her betrayal and burns both of them on a pyre. He keeps the child, which appears to set up a new cycle of vengeance.

Babu raises the child and vows to stay away from crime. One day, while going through the notebook, he sees the child has drawn a family, and his eyes widen as a gun appears in the child’s hand.

The final shot shows the child looming with a gun toward Babu, underscoring the cycle of violence. The film ends with the line What goes around comes around, leaving the audience to ponder fate and revenge.
