Reporter Carl Kolchak in Seattle is assigned by his editor, Tony Vincenzo, to cover a string of killings where exotic dancers are strangled. The coroner’s report reveals that the victims’ necks were crushed—an feat far beyond any human strength—blood was drained, and there are traces of rotting flesh around the wounds. As the case unfolds, Kolchak’s keen eye for odd details begins to pull together a pattern that others want to ignore. The crime scenes aren’t random; they feel almost ritual, and the more Kolchak digs, the more the questions multiply.

Kolchak enlists the help of researcher Titus Berry, who uncovers a chilling pattern: a similar rash of killings in 1952, with earlier cases in 1931. The pair piece together a grim cycle: murders recur every 21 years, each flare lasting about 18 days. Kolchak theorizes that the killer drinks blood to sustain an elixir of life, a cycle that would keep him alive for 21 years at a stretch, a claim that few take seriously and that the police treat as fringe sensationalism. Yet Berry’s meticulous work presses the detectives to consider a danger far beyond ordinary crime—one that seems to defy time itself.

Berry’s digging leads to a clue tied to an interview with Mark Twain, pointing toward Dr. Richard Malcolm, a Civil War-era surgeon who was involved with Seattle’s Westside Mercy Hospital. Though the hospital is long gone, Kolchak visits the site of the old clinic and discovers a painting of the founder, a figure who bears a striking resemblance to Malcolm. Kolchak calls Berry to meet him at the site, and the two men press the authorities for a candid accounting. They’re met with skepticism, and Kolchak is arrested, but the nagging sense that something supernatural rather than merely criminal is at play continues to haunt him. The police theory remains cautious, while Kolchak and Berry refuse to abandon the possibility that a nearly immortal killer could be real.

Kolchak and Louise Harper, a psychology student who works as an exotic dancer to make ends meet, push forward to stop the killer before he completes his elixir and vanishes for another 21 years. In the Seattle Underground beneath the old clinic, Kolchak uncovers the preserved ruins of Westside Mercy Hospital and finds the latest victim—a tramp he had spotted earlier on a tour of the subterranean passages. Malcolm, alerted by the disturbance, finally speaks, admitting that he first sought immortality in 1868 and that the 1889 Seattle Fire cost him his family. He claims to be refining a method to make the elixir permanent, but Kolchak remains unconvinced. When Kolchak smashes the elixir beaker, Malcolm’s tightly maintained illusion begins to crack. Without the elixir, the centuries catch up with him in a rush: his body ages rapidly toward a true age of about 147 years, his hands cramping as his strength fails. The confrontation ends with the arrival of the police witnessing Malcolm’s suicide by leaping from a high window, a moment that shatters the myth of invincibility.

Back above ground, Vincenzo redacts the supernatural elements of Kolchak’s tale, reporting only that the killer was found but his identity remains unknown. Kolchak argues with Vincenzo and storms out in a fury, and the newsroom learns that Vincenzo has been fired while Louise is expelled to quell the controversy. Unshaken and unpaid, Kolchak refuses to surrender the truth as he sees it. He drives the three of them—Berry, Louise, and himself—toward New York City, ready to chase another story in a world where legends and reality collide and every mystery might be just beyond the edge of belief.
