Jack, Ross Destiche wakes up in an old, abandoned mental institution. He is not alone; four other college students are in the same predicament. They soon realize they are part of a secret experiment conducted by the mysterious Dr. Broward, Brad Dourif.

As the group explores the looming halls, they confront hallucinations and the eerie side effects of a drug they’ve been unknowingly given. They split up, scouring the vast building for the so‑called “Blank Slate” files, which they believe hold the key to their predicament. Jack, with the help of the ghostly Anne, Emily Soto, discovers the files tucked away in a cabinet. The folders reveal patient records, each stamped with the word “Wiped.”

Meanwhile, Grant, Justen Jones, is seized by a group of plague-doctor–like figures. He endures a procedure that mutates him into a zombie‑like creature, though he later reappears with fragments of his former self intertwined with the new form. From the files, the group learns that the experiment is designed to store consciousness rather than erase it. The last consciousness stored belongs to a dangerous patient famous for horrific crimes, and they realize that this consciousness is now housed inside the machine they are tethered to.

As threats mount—the relentless zombie‑like beings, the hospital’s staff, and their own frailty of mind—the group suffers further losses. Vanessa, Jenna Enemy, and other members are dragged into the danger, and Heather, Monique Candelaria, is killed in a brutal encounter.

In the film’s climax, Jack confronts the ordeal head-on with Dr. Broward in the depths of the asylum, where a large faded vault sits sealed away. Inside lies a mottled skeleton, the remnants of the last patient subjected to the consciousness‑storing procedure. They remove the wires from the skeleton, preparing to power the machine once more. Outside, other survivors mount a rebellion, fighting off agents loyal to Broward as the institution shudders under the strain of the madness they’ve unleashed.

A shocking turn unfolds when Grant resists and lashes out against Dr. Broward, intensifying the chaos and accelerating the escape. The survivors manage to flee the hospital, leaving the unconscious doctor behind to face an uncertain fate.

With the building fading into silence, the fate of Grant—the last known subject still undergoing the same procedure—remains unresolved, leaving a lingering question about what the experiment will ultimately unleash if revisited.
