Skye Riley becomes the next host of the entity through a series of traumatic events that begin with her witnessing her boyfriend’s tragic death and her own substance abuse struggles. The entity feeds off trauma, especially unresolved pain and guilt, and it latches onto her after she experiences intense mental and emotional turmoil. Throughout the film, Skye’s reality becomes increasingly blurred as she struggles with hallucinations, messages from an unknown sender, and memories that may or may not be real. Much of what she perceives—like her conversations with people who aren’t really there, or events that seem to happen in her mind—is part of her mental breakdown and the influence of the entity.

By the climax, Skye’s perception is completely shattered. She attempts to follow the plan to die to break the cycle, as advised by Morris, but ultimately discovers that much of what she experienced—her hallucinations, her confrontations, even the wellness center scene—were illusions or hallucinations created by her fractured mind. The true reality is simpler: she is still alive and surrounded by the people who think she’s doing well. The final moments reveal a haunting image: Skye, on stage, seemingly recovered and performing for her fans, but with a grotesque twist. She rips open her stomach, symbolizing her inability to heal from her trauma, and the monstrous entity then emerges from her body. The scene shifts to her giving a chilling smile amidst horror as she appears to have fully become the vessel for the entity, which then takes control. The ending leaves it ambiguous whether Skye is truly free from the cycle or if she has succumbed entirely to the entity’s influence. It hints at a potential sequel, as her smile becomes disturbingly fixed and her eyes reflect a loss of her true self, suggesting that her trauma has become an inescapable part of her. The film closes with the horrifying spectacle of Skye’s transformation, emphasizing how trauma and mental health struggles can be deeply intertwined with evil forces, leaving the story open-ended and unsettling.
