Father Jedediah Mayii performs an exorcism and banishes the demon from Nancy Aglet. The ritual leaves Nancy relieved, but Mayii and Nancy are shown to be weakened by the effort, foreshadowing a return of the possession.

In 1990, Nancy's body is possessed again while she is watching The Ernest and Fanny Miracle Hour, a televangelist program. The sudden possession triggers concern among those nearby and sets the plot in motion.

Following the hospital visit, Father Luke Brophy assesses Nancy and concludes she is possessed again. His diagnosis prompts consideration of an exorcism and mobilizes the plan for intervention.

Mayii refuses to perform the exorcism, claiming he is too weak and that both he and Nancy barely survived the previous attempt. He insists that the rite cannot be safely conducted at that moment.

Brophy travels to the Supreme Council for Exorcism Granting with Ernest and Fanny to argue that an exorcism is warranted. They secure permission to televise the ritual, believing it will reach millions.

The council approves the televised exorcism and Ernest and Fanny announce Ernest and Fanny's Exorcism Tonight to the network.

Seeking strength, Mayii visits Bods-R-Us gym to regain his physical condition. Brophy approaches him there and informs him about the televised plan, but Mayii remains reluctant.

A montage shows attempts to free Nancy through phone donations, singing, and insults from supporters, gradually building the largest audience in history for the broadcast. The growing spectacle heightens the stakes and the sense of inevitability around the exorcism.

During the broadcast, the devil in Nancy's body sets the studio on fire and reveals that he manipulated the setup to attract a massive crowd. He also turns Ernest and Fanny into a pantomime horse as part of the revelation.

The devil attempts to claim the souls of the viewing audience via the studio camera, but Brophy intervenes by destroying the camera. His action interrupts the possession attempt and shifts the momentum of the confrontation.

With the camera destroyed, the devil heads for a satellite transmitter, and religious figures from around the world gather at Brophy's command to pursue him.

Back in the studio, the devil uses the camera to lure Mayii to him for a rematch, reigniting the confrontation. Pressure mounts as past defeats haunt Mayii and the fate of Nancy hangs in the balance.

The exorcism is presented on screen with commentators Mean Gene Okerlund and Jesse Ventura, as the ritual struggles to reclaim Nancy. The devil proclaims his hatred of rock and roll during the exchange.

The studio becomes a live concert as religious figures including the Pope perform in an effort to torment the devil. They play the song Devil with a Blue Dress On on screen, and the exorcism ends with the devil being driven from Nancy's body for good.
