In 1972, two priests are summoned to Jerusalem to perform an exorcism on a woman who was thought to have died three days prior. Despite initial calmness, the woman becomes violent and injures her husband. The priests are left with no choice but to kill her in a desperate final attempt to save her.

In the present day, two Jewish Americans, Rachel Klein and Sarah Pullman, board a flight to Tel Aviv. They meet a student named Kevin Reed who is fascinated with religious myths, particularly those surrounding dark angels and other supernatural beings.

Kevin invites Rachel and Sarah to join him on a journey to Jerusalem, where he wants to spend Yom Kippur. Sarah develops a crush on Kevin and persuades Rachel to accompany them on this adventure.

Upon arriving in Jerusalem, the friends encounter a local who believes he is King David. He urges them to leave the city immediately due to its dangerous aura, but they dismiss his warnings as insanity.

The group meets Omar, a local Palestinian, at their hotel. Rachel begins an affair with Omar while Kevin and Sarah's bond deepens as they explore the city together. Their time in Jerusalem is marked by joy as they party and visit historical sites.

At the Western Wall, Sarah makes a heartfelt wish for her deceased brother Joel to return to her. As her prayer is cast into the wall, an ominous swarm of black birds erupts into the sky, signaling a strange shift in the atmosphere.

The group decides to explore Solomon's Quarries, where Kevin becomes increasingly anxious after noticing eerie drawings on the walls. He urges Sarah to investigate the archives of the city for answers, suspecting that something is terribly wrong.

After his research, Kevin insists that Sarah should leave Jerusalem immediately, foreseeing a catastrophe ahead. Although searching the city feels off to Sarah, Rachel is determined to stay and enjoy their time.

As night falls, Sarah witnesses military planes bombing parts of Jerusalem, which leads to media reports of a terrorist threat. In a panic, she rushes to free Kevin from a mental asylum where he has been confined.

Despite Sarah's efforts to rescue Kevin, she is met with resistance from Rachel and Omar, who are escorted by Israeli soldiers. Ultimately, Sarah convinces the soldiers to help her free Kevin, leading to a chaotic escape.

Once united, the friends learn that Jerusalem is now sealed off and under quarantine. The city walls are blocked, and anyone attempting to leave is shot, forcing the group to consider alternative escape routes.

Omar's father reveals a way to escape through underwater caves he remembers from his childhood. The group decides to take this risk, but unknown dangers lurk in the shadows, threatening them on their journey.

As they navigate the caves, they are violently attacked by demons that infect, bite, and consume anyone in their path. Tension rises as Kevin realizes that the gates of hell have been unleashed upon the city.

Inside the cave, Rachel succumbs to her infection and her physical transformation begins. In a moment of clarity, she warns Sarah to flee before ultimately taking her own life to save her friends from her changing nature.

At the climax, Sarah meets a creature with black wings, who reveals himself to be her resurrected brother Joel. Just as they escape, Sarah discovers her own infection, resulting in her transformation into a demon as Kevin watches helplessly.
