Misty Calhoun and her students Leslie and Daniel Kim are invited to the sea-based facility Akhelios by billionaire Carl Durant. They travel by motorboat with Durant's associate Craig Burns and are greeted by Durant and the crew. The visit sets up the investigation into the facility's shark experiments.

Durant demonstrates his control over the sharks by throwing Aaron into the water and letting the five genetically altered bulls chase him. At the last moment, Aaron uses a device to drive the sharks away in formation, impressing Durant but unnerving Misty. The scene establishes the ethical and safety concerns of the experiments.

In the wet lab, Durant notes that alpha shark Bella has been acting strangely. Misty suspects she is pregnant, implying the genetic alterations may have masked a natural pregnancy. Durant is puzzled and cautious about the cause of her behavior.

Durant brings Bella into the wet lab and has Josh take a saliva sample. Bella struggles and nearly bites him, underscoring the danger of the modified sharks. The scene advances the tension around the experiment's risks.

On the surface, a second shark drives the motorboat into an electric control box, causing an explosion and significant damage. The attack also shuts down several facility systems, allowing the sharks to flood the building. Craig is killed in the chaos.

Bella goes into labor after being dropped back into the pool, and she gives birth to a litter of shark pups. The birth is linked to the earlier genetic experiments and signals the facility's growing danger. The pups immediately pose a lethal threat to those inside.

Mike Shutello attempts to swim to the surface but is knocked unconscious by a shark. Trent Slater saves him, and Misty revives him, but as Mike leans over to recover his gear, a shark beheads him. The incident marks the collapse of the facility's internal safety.

A lack of pressure stabilization allows water to rush into the flooded rooms, separating the survivors into smaller groups: Misty and Trent, Aaron and Josh, and Durant with Daniel. Leslie is knocked unconscious in the chaos as the others scramble for survival.

Trent reveals that Durant has been treating himself with a genetics-altering liquid to become more intelligent, driven by a fear that technology will dominate humanity. Durant briefly reunites with Misty and Trent before continuing toward escape. The revelation elevates the tension and mistrust among the group.

Misty, Trent, and Durant reach an air ventilation shaft that would lead them to the surface. Durant locks Misty out of the room after she tries to search for other survivors, enraging Trent. Despite the strain, Misty and Trent manage to reach the surface together.

Aaron and Josh reach one of the sleeping quarters, but shark pups find them and kill Josh. Aaron escapes on an air mattress, narrowly avoiding the predators. The attack highlights the pups' deadly aggressiveness.

Daniel finds Leslie behind a door, and the shark pups kill her in front of him. He is left shocked and later encounters Aaron as they search for a way to the surface. The tragedy marks the escalation of the pups' threat.

Misty uses a blow torch to gain entry to the room with the ventilation shaft and is met by Aaron and Daniel. The pups attack, but Misty distracts them while Aaron and Daniel climb toward the surface. Misty eventually reunites with the others on the surface using scuba gear.

Realizing the facility is sinking, the survivors race for the motorboat. Aaron is dragged underwater by a shark while Bella confronts and kills Durant. Misty and Trent reach the motorboat, using flare guns to kill a shark, and Aaron joins them aboard. Durant triggers the self-destruct, destroying much of the facility and killing many sharks offscreen.

In the aftermath, Bella and many of her pups survive the explosion and are now free in the open ocean. Misty, Trent, and Aaron escape in the motorboat, but the threat of the intelligent sharks remains a looming menace for the world.
