Basket Case

After falling from a hotel window at the end of the first film, Duane Bradley and his conjoined twin brother Belial are taken to the hospital. Their unusual and deformed appearance draws media attention, making it impossible for them to live a secret life. They are soon rescued by Granny Ruth after her seeing their story on the news.

Granny Ruth takes Duane and Belial to her home, where she and her granddaughter Susan care for a family of similarly deformed individuals. Among them is Eve, a bodyless torso, who is traumatized and mute due to her past treatment and keeps to herself in the attic. The extended family forms a fragile community of outcasts.

Over the course of several years, Eve and Belial develop a romantic relationship, while Duane's resentment toward Belial intensifies. Duane remains haunted by his past and his sister Sharon's murder, fueling his desire to distance himself from the 'freaks.'

Sleazy reporter Marcie and photographer Arty search for the Bradley brothers to expose them. Their efforts lead them to discover the family of deformed individuals, prompting plans to reveal their secrets to the world. Ruth and her family try to protect their secret from Marcie and Arty.

Marcie, Arty, and private detective Phil attempt to capture the brothers for their story. The family defends themselves, resulting in Ruth and her family killing Arty and Phil to stop their exploitation. Belial mutilates Marcie’s face, transforming her into a freak in retribution.

That night, the family celebrates their victory over the media intruders. Meanwhile, Eve and Belial consummate their relationship in the attic, deepening their emotional bond amidst the chaos. The family’s unity is reinforced by their shared defformity.

Seeing an opportunity to escape his brother’s control, Duane confides in Susan and asks her to run away with him. Susan reveals she is also a freak, pregnant with a grotesque creature emerging from her surgical wounds, shocking Duane and shattering his psyche.

When Susan’s grotesque creature emerges, she is horrified, and Duane, overwhelmed by despair, pushes her out a window, killing her. This act marks a turning point for Duane, plunging him further into heartbreak and madness.

In a moment of madness, Duane sews Belial to his body, attempting to become one with his brother again. This act signifies Duane’s complete loss of grip on reality and his desire for connection at any cost.

Ruth and the others find Duane, now grotesquely fused with Belial. They are horrified at what he has done, while Duane insists that he and Belial are 'together again,' confirming his descent into madness and the tragic culmination of his emotional turmoil.
