Peter, being the prodigal grandson of camp owner Gilbert McCalister, persuaded the counselors to perform the blood ritual because he believed in the living legend of Nurse Agatha. According to the camp's lore, Agatha had haunted the grounds forty years earlier with occult experiments until she was forcibly stopped. Peter likely wanted to prove the legend was real or wanted to contact her, not realizing the ritual would actually work and unleash genuine evil.

During the blood ritual performed at the end-of-season party, the ceremony was successful and Agatha was able to possess Danny, one of the counselors. The ritual served as a conduit for Agatha's spirit to enter the physical world through the participants. Once possessing Danny, she used him as a vessel to carry out violence, immediately causing him to bludgeon Kellie to death.

Decades before the events of 1987, Agatha came to Camp Briarbrook and made a dark proposition to Gilbert: she promised to heal his wife Evelyn, but instead poisoned her. When Gilbert discovered this deception and betrayal, he murdered Agatha in a fit of rage. He buried Agatha in the deep forest with Evelyn's wedding ring on her finger, sealing the dark secret that would haunt the camp for forty years.

During the group's search for missing counselors Ben and Ashley, Mike discovered Veronica bound to a bed with her tongue cruelly cut out. Ashley was also found dead, killed and devoured by the possessed children.

The film ends with Lauren looking on in horror as Agatha's malicious laughter echoes across the camp, implying that despite the survivors dismembering her and casting her remains into the lake, Agatha's evil may not be truly defeated. The ambiguous ending suggests some evils, once summoned, refuse to stay buried. The ominous implication is that Agatha's return is inevitable, leaving the survivors to face the haunting possibility that the nightmare could begin again.

After the ritual went wrong and the camp descended into terror, she attempted to maintain order in the lodge as the power failed and danger closed in. She was present during the confrontation where Peter learned the truth about Agatha, Evelyn, and Gilbert, and witnessed the subsequent chaotic events unfold.
