Counselors at Camp Briarbrook gather for an end-of-season party, and Peter presses them to perform a blood ritual to summon Nurse Agatha. The legend says Agatha terrorized the camp forty years earlier with occult experiments. The group goes ahead, hoping to contact her.

The ritual proves successful as Agatha possesses Danny and several children. The possessed behave with eerie calm, then turn increasingly violent. The camp's counselors realize they have unleashed a deadly force.

In a brutal outbreak of violence, possessed Danny bludgeons Kellie to death in front of the others. Mike fights back in self-defense and kills Danny to stop the attack. The group reels from the sudden, deadly turn of events.

The possessed children return to the camp and perform a dark resurrection, bringing Agatha back to life. Fear grips the counselors as the legend steps back into the world. The camp braces for what comes next.

The group retreats to the main lodge, but a sudden power outage plunges the camp into darkness. Heather attempts to call the police, and in the chaos she is injured. The silence of the lodge deepens the sense of danger.

Mike, Veronica, and Dylan go to locate the missing Ben and Ashley. During the search, Dylan's scare causes Veronica to flee in fear. The trio's mission exposes the fragility of the adults in the face of supernatural threats.

Peter confronts his mother Heather, Gilbert, and Shawn returns, and learns Agatha's origin. Agatha came to heal Evelyn but became obsessed with Gilbert and poisoned Evelyn; Gilbert discovered the truth and murdered Agatha, burying her body in the forest. The resurrected Agatha now seeks to reclaim Evelyn's wedding ring.

Mike and Peter devise a plan to trap the children in the basement. Mike lures them inside and escapes, but Dylan knocks him back into the swarm and the children kill him.

They use Gilbert and Evelyn's ring as bait to coax Agatha out. Dylan tortures Ben and leaves him as an offering for Agatha; Shawn, Peter, and Lauren rush to save Ben, but Peter is killed by Agatha.

The plan works briefly as Agatha is drawn out, but she becomes enraged when the ring is revealed as fake and she kills Gilbert. The survivors attack and defeat Agatha, dismembering her and tossing the pieces into the lake to prevent her return.

An officer arrives the next morning, summoned by a surviving Ben. Lauren watches in horror as Agatha laughs, implying that the haunting may not be truly over.
