Katherine, a former TV star, wakes up in a hospital bed with her body entirely paralyzed and unable to speak, a condition known as locked-in syndrome. A compassionate nurse named Nicky steps in to help her regain some degree of bodily control, working patiently as Katherine learns to communicate in new ways.
To understand how Katherine arrived at this state, Nicky seeks information from Lina, the wife of Katherine’s deceased stepson, Jamie. Through a series of flashbacks, the film slowly reveals the tense history that led to the accident, intertwining love, loyalty, and betrayal.
Katherine’s husband was very wealthy, and after his death the estate passed to Jamie. Jamie longed for Katherine’s affection and care, but Katherine often felt resentment over being disinherited and deprived of a final say in her own life. Lina emerges as a central figure in this web of relationships. Lina, the daughter of a childhood friend whom Katherine had taken in, grew close to Jamie and became his main caregiver. She and Jamie eventually marry, deepening Katherine’s bitterness over Lina’s grip on the family’s wealth and estate.
Dr. Robert Lawrence, the family physician, enters the picture with a more troubling motive. He begins an affair with Lina, who is wrestling with a sense of purpose and autonomy in her constrained life. Robert’s ulterior goal, however, is to gain control of the property. He manipulates Jamie’s health by prescribing painkillers that worsen his condition, and he persuades Jamie to join him and Lina on a boat ride. On the water, Robert intentionally capsizes the boat and drags Jamie beneath the waves, despite Lina’s desperate attempts to save him.
After Jamie’s funeral, Robert presses Lina to sell the mansion so they can move away. Lina resists, holding fast to a promise she made to Katherine and recognizing Robert’s true intentions. The situation becomes more dangerous when Robert reveals he has Lina’s journal, which contains fantasies about killing Jamie and hints that Lina could be blamed for Jamie’s death. In a pivotal turn, Lina returns home only to discover that Katherine and Robert have been carrying on an affair as well. She decides to leave, but is forcibly driven out by Robert—along with Katherine. In the moment of flight, Lina witnesses Katherine raise a rifle and aim at her, a shocking sign of the volatility surrounding the mansion and its secrets.
As Katherine makes gradual progress in recovery, Nicky teaches her to communicate again by blinking to respond to letters of the alphabet. When Katherine spells the word “murder,” Nicky realizes a deeper truth about the danger surrounding Lina. Lina, learning of these developments, enlists Robert’s help to bring Katherine home, hoping that Katherine’s testimony will implicate them both in Jamie’s death.

In a tense confrontation, Lina outsmarts Robert and administers a lethal drug she had prepared for Katherine—but it is not strong enough to kill him. Robert retaliates by attacking Lina and attempting to administer another injection to Katherine. Lina defends herself, stabbing Robert to death in the struggle.
The authorities eventually arrive at the mansion and remove Robert’s body as Lina, Katherine, and Nicky watch.
