A businessman asks his friend to resolve his daughter's problem. They visit the nearby Lord Narasimha temple to seek help, but when the spirit resists, Russian exorcists are called in. The exorcism succeeds and the violent spirit is hammered into a tree with two metal nail crosses.

Raghava's family sets out for Coimbatore to celebrate the grandparents' 60th anniversary. On the way they stop in a field for a picnic. A gust of wind disturbs the ground and the metal nail crosses twist from the earth, landing in the picnic basket.

At the grandparents' house, daytime and nighttime events grow increasingly eerie as the family experiences strange, frightening incidents. The occurrences hint at a dangerous supernatural presence haunting the home. The relatives witness escalating disturbances during the celebration.

During a visit to the Muneeshwaran temple, an aghori presents a holy trishula to plant for the exorcism. The group prepares to perform the prescribed rites to drive out the spirit.

That night the spirit drags Priya from the house, and the trishula lands before her. The aghori provides three dolls to test for a ghost, ghoul, or vampire, and the family divides tasks to carry out the rituals.

During the first ritual, a black horse crashes through the house and drags Raghava's brother away, heightening fear. The ritual proceeds as the family braces for the supernatural onslaught.

In the second ritual, a herd of bulls invades the house, terrifying the family further. The violent intrusion tests the exorcists' resolve and intensifies the haunting.

The dolls' heads are ripped in the final ritual, but the ghost eats one head and continues to scare the family. The aghori eventually returns home and appears to have captured the spirit, though his claim is questioned.

One night Priya kisses Raghava, and the ghost possesses him after he looks into the picnic basket. The next night the possessed Raghava exhibits effeminate behavior, signaling a deeper level of possession.

The possessed Raghava, controlled by the spirit, brutally stabs Shankar's assistant and slaps Raghava's brother and sister-in-law. The violence underscores the danger now facing the entire family.

The mother reveals that two spirits possess Raghava, and the grandfather calls the aghori. The aghori arrives to confront the haunting, hinting that he lied earlier for a higher purpose but will still help.

The aghori explains Kaali's flashback, linking the haunting to Kaali, a social worker who runs an ashram for the underprivileged. The narrative shifts to the past timeline to reveal Kaali's backstory.

In Kaali's past, Bhavani, the minister's younger brother, asks Kaali to store ₹100 crores in his ashram. Kaali refuses, sparking a deadly feud that leads to a brutal series of murders and public support for Kaali.

Kaali orchestrates revenge after Bhavani's murder, and Rosie dies on a bike during a fateful ride. Kaali later dies while watching a video showing the Radhaamma ashram burning, an act orchestrated by Shankar as part of a larger plot against him.

In the present, Goddess Kali enters Kaali. Kaali defeats the Russian exorcists and kills Shankar, avenging Rosie. Kaali and Rosie return Raghava to his family and depart, bringing a fragile peace to the household.
