Ebola Syndrome

In 1986 Hong Kong, Kai San sleeps with his boss Kwan's wife. When Kwan catches them, the wife accuses Kai of assault. In a violent confrontation, Kai kills Kwan, Kwan's wife, and a henchman, and almost kills Lily before fleeing as a wanted man.

Kai becomes a wanted convict and escapes from Hong Kong to South Africa. He attempts to start anew while avoiding capture. The move marks the end of his initial violent spree and the start of a new life.

Ten years after the Hong Kong murders, Kai works as a chef in a Chinese restaurant in South Africa. His boss Kei and Kei's wife bully him, creating a tense, oppressive workplace. The simmering resentment sets the stage for Kai's later violence.

Lily, Kai's former victim's daughter, is now a young flight attendant who visits Kai's restaurant. She becomes ill when in his proximity, hinting at a connection between Kai's past and the illness spreading. The encounter marks a personal anchor to Kai's shadowy history.

Kei takes Kai to a Zulu tribe to obtain cheap pigs for the restaurant. The tribe performs a ceremony around sick members, highlighting the ominous atmosphere. Kei buys the pigs, signaling the start of the gruesome events to come.

On their return, Kai crashes the car into a tree. While Kei tends to the car, Kai discovers a dying sick tribe member and rapes and kills her. The act foreshadows the Ebola-related violence that follows.

Kai develops a fever and is diagnosed with Ebola. He is immune to the disease, turning him into a living carrier who can spread the virus through bodily fluids. The infection explains the outbreak ripple that follows.

While Kei is away, Kai overhears his wife plotting to kill him and retaliates by killing her. He rapes her and gouges out her eyeball, then kills her cousin. He dismembers the corpses and serves them to restaurant customers as hamburgers, initiating a terrifying chain of events.

Lily and her boyfriend return to the restaurant, and she recognizes Kai as the killer from her childhood. They report to the police, but authorities lack evidence to act. The escalating outbreak underscores Kai's continued danger.

Using Kei's money, Kai returns to Hong Kong and sleeps with two prostitutes, both of whom fall ill. The case serves as a bridge between Africa and Asia, widening the outbreak and making Kai harder to track. The pursuit now crosses continents.

South African and Hong Kong police—the collaborating effort alongside Lily—work to identify and locate the mysterious virus spreader. Evidence links the outbreak to Kai's movements and the acts he committed, furthering the chain of infection. The investigation intensifies as more victims appear.

Kai reconnects with his ex-girlfriend Har in Hong Kong, who now has a husband and daughter. He buys Har from her husband, and rekindles his assaultive behavior by spreading the virus to many, including Lily, as part of his manipulative cycle. The relationship turns increasingly deadly as Har learns of his disease.

Kai has sex with Har and is spurned by her when she learns of his disease. He grabs Har's daughter hostage, spits in people's faces, and fights to escape custody. In the chaos, he strangles Har's daughter to death, a turning point that provokes a final purge.

Inspector Yeung tracks Kai to a final confrontation. He sets Kai on fire and shoots him repeatedly, killing him and ending the outbreak. The city begins to recover as the truth about Kai's reign of infection comes to light.
