Dr. Benjamin Stone, portrayed by Michael J. Fox, is a talented young surgeon who’s tired of the monotonous emergency room life in Washington, D.C. Eager for a change, he dreams of becoming a plastic surgeon in Beverly Hills to pay off his med school debts while enjoying a more glamorous lifestyle. However, on his final day at the hospital, he senses a lack of warmth from his colleagues. To add insult to injury, a farewell cake is humorously defaced with “Good riddance, asshole.”

His grand plans take a detour when a cross-country drive in his classic 1956 Porsche turns disastrous, landing him in the quirky small town of Grady, South Carolina, after crashing into the fence of local Judge Evans (Roberts Blossom). His humble offer to repair the fence backfires as the irate judge assigns him 16 hours of community service at a nearby hospital, which escalates to 32 hours due to his growing frustration. As he arrives at the hospital, the uncompromising Nurse Packer (Eyde Byrde) swiftly puts him in his place, treating him like a regular worker with a strict clock-in and clock-out regime.

Despite his initial resistance, Ben forms a bond with the town’s charming Mayor, Nick Nicholson, played by David Ogden Stiers, who sees potential in Ben to fill the shoes of the aging local doctor, Aurelius Hogue, portrayed by Barnard Hughes. During a warm welcome at the mayor’s lakeside lodge, featuring homemade hospitality from café owner Frances Sternhagen, Ben meets the spirited ambulance driver, Vialula “Lou,” played by Julie Warner. Amidst his community service, Ben becomes entangled with the mayor’s flirtatious daughter, Nancy Lee, played by Bridget Fonda, who quickly takes an interest in him.
